KORELASI DELTA NETROFIL INDEX (DNI) DENGAN JUMLAH GRANULOSIT IMMATUR PADA KECURIGAAN INFEKSI BAKTERIAL

Penghitungan leukosit, termasuk neutrofil mempunyai peran penting pada pasien dengan suspek infeksi, karena perannya dalam melawan infeksi bakteri akut. Manual "count band" digunakan secara luas sebagai penanda untuk infeksi bakteri mempunyai keterbatasan pengukuran secara akurat dan tepat dimana hasil yang diperoleh tergantung subjektifitas observer. Automatic cell analyzers (yaitu, ADVIA 120, Siemens, Inc) menganalisis hitung jenis lekosit dengan metode reaksi sitokimia myeloperoxidase (MPO) dan nuclear loburity channel. Dalam penelitian ini, delta netrofil indeks (DNI) merupakan selisih antara lekosit yang diukur pada MPO channel dengan nuclear lobularity channel yang mencerminkan fraksi granulosit imatur di dalam sirkulasi darah. Delta netrofil index berkorelasi positif dengan jumlah granulosit imatur pada kecurigaan infeksi bakterial. Tujuan dari penelitian ini adalah untuk mengetahui korelasi DNI dengan jumlah granulosit imatur dan bagaimana agreement hasil keduanya. Penelitian ini menggunakan rancangan potong lintang. Sembilan puluh empat subjek dengan kecurigaan infeksi disertai peningkatan jumlah lekosit dilakukan pemeriksaan DNI menggunakan ADVIA 120 dan penghitungan jumlah granulosit imatur manual, kemudian dilakukan uji korelasi serta plot Bland Almant untuk mengetahui agreement hasil kedua metode pemeriksaan. Hasil penelituan ini menunjukkan bahwa Nilai DNI berkorelasi positif dengan hitung granulosit imatur manual dengan koefisien korelasi (r) sebesar 0,871 (95% CI: 0.812 - 0.913), dengan bias (mean of difference) sebesar -3,5.

Leukocytes counts, include neutrophils play an important role in patients with suspected infection, because of its role in against acute bacterial infections. Manual "band count" is widely used as a marker for bacterial infection have limited measurements accurately and precisely where the results are obtained depending on the observer subjectivity. The delta neutrophil index (DNI), the difference between the leukocyte differentials computed in the MPO channel and those calculated in the nuclear lobularity channel in the automatic cell analyzer (ADVIA 120) that reflects the fraction of circulating immature granulocytes in the blood. Delta neutrophil index correlated positively with immature granulocyte. The purpose of this study was to determine the correlation between DNI and immature granulocytes count and how agreements between those methods. Ninety-four subjects with suspected infection accompanied by an increase in the number of leukocytes were studied. DNI (the difference in leukocyte subfractions identified by myeloperoxidase and nuclear lobularity channels) was determined using ADVIA 120 then performed a correlation test and Bland plot Almant to know the agreement between two methods. The study results indicate that the DNI value correlated positively with immature granulocyte count manually with a correlation coefficient (r) 0.871 (95% CI: 0812-0913), and the mean of difference -3.5.
