Evaluasi Kesesuaian Resep Dengan Formularium Pada Pasien Umum Rawat Jalan Di RSUD Tidar Kota Magelang

Penggunaan formularium dapat menjamin standar peresepan yang berkualitas baik dan cost-effective. Peresepan yang berkualitas dan cost effective bertujuan untuk mewujudkan penggunaan obat yang rasional. Akan tetapi di RSUD Tidar Kota Magelang masih banyak dokter yang menulis resep diluar obat-obatan yang ada di formularium sehingga banyak resep yang tidak terlayani di apotek yang ada di RSUD Tidar Kota Magelang. Berdasarkan permasalahan di atas, maka perlu dilakukannya evaluasi kesesuaian resep dengan formularium rumah sakit. Tujuan Penelitian: Untuk mengetahui prosentase kesesuaian resep dengan formularium berdasarkan nama zat aktif dan nama dagang obat pada pasien rawat jalan di RSUD Kota Magelang, mengetahui presentase obat yang tidak terlayani, serta mengetahui motivasi dokter untuk menuliskan resep sesuai dengan formularium. Metode: penelitian ini merupakan penelitian case study yang bersifat deskriptif analitik. Hasil Penelitian: Bulan Januari-Maret terdapat 99,05 % kesesuaian resep dengan formularium berdasarkan nama zat aktif, 75 % kesesuaian resep dengan formularium berdasarkan nama dagang obat, dan 35,6 % obat yang tidak terlayani. Bulan April -Juni terdapat 99,4 % kesuaian resep dengan formularium berdasarkan nama zat aktif, 68 % kesesuaian resep dengan formularium berdasarkan nama dagang obat, dan 50,3 % obat yang tidak terlayani. Bulan Juli-September terdapat 98,9 % kesesuaian resep dengan formularium berdasarkan zat aktif obat, 67,4 % kesesuaian resep dengan formularium berdasarkan nama dagang obat, dan 44 % obat yang tidak terlayani. Bulan Oktober-Desember terdapat 99,3 % kesesuaian resep dengan formularium berdasarkan zat aktif obat, 68,7 % kesesuaian resep dengan formularium berdasarkan nama dagang obat, dan 38,8 % obat yang tidak terlayani. Motivasi dokter untuk mematuhi formularium juga kurang karena tidak di dukung oleh rumah sakit. Kesimpulan: resep yang ditulis dokter di RSUD Tidar Kota Magelang belum memenuhi dengan pedoman pengobatan yang digunakan untuk pasien umum rawat jalan yaitu formularium rumah sakit.

Background: The use of formulary prescribing standards that can guarantee good quality and cost-effective. Prescribing quality and cost effective aims to achieve rational use of medicines. However, in Tidar Magelang public hospital still many doctors who prescribe medicines beyond existing formulary so many recipes that are not provided in the existing pharmacy in hospitals Tidar Magelang. Based on the above problems, it is necessary to the evaluation of the suitability of the recipe with the hospital formulary. Objective: To determine the appropriateness of prescribing with formulary percentage based on the name of the active substance and the drug trade name on an outpatient basis in hospitals Magelang, find out the percentage of drugs that are not provided, as well as knowing the motivation for doctors to prescribe in accordance with the formulary. Methods: This research is a case study which is descriptive analytic study. Result: January to March are 99.05% conformity with the prescription formulary based on the name of active substance, 75% conformity with the prescription drug formulary by trade name, and 35,6% of the drugs that are not served. Months from April to June are 99.4% by name formulary prescription with the active substance, 68% conformity with the prescription drug formulary by trade name, and 50.3% of the drugs that are not served. Months from July to September are 98.9% conformity with the prescription drug formulary based on active substances, 67.4% conformity with the prescription drug formulary by trade name, and 44% of drugs that are not served. Months from October to December are 99.3% conformity with the prescription drug formulary based on active substances, 68.7% conformity with the prescription drug formulary by trade name, and 38,8% of the drugs that are not served. Motivation physicians to comply with formulary is also less because it is not supported by the hospital. Conclusion: doctor's prescriptions written in Tidar Magelang public hospital not meet the guidelines that are used for the treatment of public patients are ambulatory hospital formulary.
