PERANCANGAN PROSEDUR KERJA DAN INSTRUKSI KERJA UNIT REKAM MEDIS DI PUSKESMAS GONDOKUSUMAN II YOGYAKARTA

Latar Belakang :Rekam Medis merupakan dokumen penting dari seorang pasien yang berisi riwayat sakit dari pasien. Sehingga membutuhkan suatu pengelolaan dan penyelengaraan rekam medis dengan baik dan sesuai dengan standar prosedur (prosedur kerja dan instruksi kerja). Pengelolaan dan penyelenggaraan rekam medis dilakukan oleh petugas rekam medis (perekam medis) ataupun petugas administrasi di suatu pelayanan kesehatan. Pada kasus yang perancang ambil yaitu mengenai masalah tidak adanya beberapa prosedur kerja dan instruksi kerja pada unit rekam medis yang menyebabkan beberapa petugas rekam medis mengerjakan tugasnya sesuai dengan persepsi masing – masing. Di Puskesmas Gondokusuman II Yogyakarta juga akan mengadakan sertifikasi ISO 9001 : 2008 dimana salah satu syarat sertifikasi adalah adanya prosedur kerja dan instruksi kerja. Dengan demikian maka perancang ingin membantu untuk merancang prosedur kerja dan instruksi kerja unit rekam medis yang belum ada yaitu pada pelaporan, koding, pembuatan SKM dan penyusutan berkas rekam medis, sedangkan yang sudah ada adalah prosedur kerja dan instruksi kerja pada pendaftaran dan filing. Tujuan: Merancang prosedur kerja dan instruksi kerja unit rekam medis di Puskesmas Gondokusuman II Yogyakarta. Metode Perancangan : rancangan dilakukan dengan menganalisis kegiatan rekam medis kemudian merencanakan kebutuhan perancangan prosedur kerja dan instruksi kerja setelah itu mendesain rancangan dan tahap terakhir adalah perwujudan rencana kebutuhan melalui penerimaan rancangan prosedur kerja dan instruksi kerja. Hasil: Rancangan yang sudah di buatantara lain Prosedur kerja pengkodean penyakit, Prosedur kerja penyusutan berkas rekam medis, Prosedur kerja pembuatan Surat Keterangan Medis, Prosedur kerja Pelaporan SP2TP, Instruksi Kerja Entry data pemeriksaan ke simpus, Instruksi Kerja Penulisan Kode Diagnosa ke Berkas Rekam Medis, Instruksi Kerja Pembuatan Surat Keterangan Sakit, Instruksi Kerja Pembuatan Surat Keterangan Sehat, Instruksi Kerja Pembuatan Surat Keterangan Kematian, Instruksi Kerja Pembuatan Surat Bebas NAPZA, Instruksi Kerja Pembuatan Daftar Pertelaan Penyusutan, Instruksi Kerja Pembuatan Berita Acara Penyusutan, Instruksi KerjaPembuatan LB 1, Instruksi Kerja Pembuatan LB 2, Instruksi KerjaPembuatan LB 3, Instruksi KerjaPembuatan LB 4, Instruksi Kerja W1, Instruksi Kerja W2, Instruksi Kerja LT 1, Instruksi Kerja LT 2, Instruksi Kerja Pembuatan LT 3. Adanya rancangan ini pelayanan rekam medis pada koding, pembuatan surat keterangan medis, pelaporan dan penyusutan berkas rekam medis di Puskesmas Gondokusuman II Yogyakarta akan mengikuti peraturan yang berlaku sesuai standar yang telah disepakati.
