Latar Belakang : rekam medis merupakan ringkasan fakta-falta sejarah kehidupan dan kesehatan pasien,termasuk penyakit lama dan penyakit sekarang serta pengobatannya,ditulis oleh profesional kesehatan yang ikut mengasuh pasien tersebut (Huffman,1994) .Berdasarkan pengertian diatas dapat digambarkan bahwa rekam medis merupakan sumber pelepasan informasi kesehatan pasien selama mendapatkan pelayanan. Untuk menunjang kebutuhan pencatatan rekam medis diperlukan fasilitas atau sarana pencatatan yang baik agar tercipta rekam medis yang baik. Isi rekam medis yang sesuai dengan kebutuhan akan sangat membantu dan memudahkan proses perekaman informasi, maka dari itu diperlukan formulir yang sesuai kebutuhan dalam isi rekam medis. Selain melihat formulir dari segi isi rekam medis, dalam pelayanan rawat jalan membutuhkan formulir yang memperhatikan aspek fisik, anatomik, isi dan dari aspek fungsi rekam medis tanpa mengurangi informasi yang seharusnya dimuat. Tujuan Perancangan : Merancang ulang bentuk formulir rekam medis rawat jalan yang sesuai kebutuhan dengan memperbaiki item-item yang ada didalamnya dan difokuskan pada aspek anatomik, isi, dan fisik formulir guna mempermudah dan mempercepat proses pelayanan tanpa mengurangi fungsi dasar rekam medis. Proses Perancangan : perancangan dimulai dengan pengumpulan data yang berkaitan dengan formulir rekam medis rawat jalan, menentukan teori perancangan formulir yang digunakan. Perancangan dengan membuat sketsa alternatif formulir, membuat gambar rancangan, mewujudka rancangan lalu dilakukan pemilihan dan uji coba formulir. Hasil Perancangan : rancangan rekam medis rawat jalan yang dihasilkan mengacu pada kebutuhan pengguna dan fokus untuk menyederhanakan, serta mempermudah penggunaan formulir dan memperbaiki kualitas kertas yang lebih kuat.

The purpose of design: Redesign the form record the appropriate outpatient medical needs by improving existing items in it and focused on aspects of anatomik, contents, and physical form in order to simplify and speed up the process of service without compromising the basic functions of the medical record. Design process: the design starts with the collection of data relating to outpatient medical record form, define the theory of the design of the form used. Design by sketching out an alternative form, to create an image design, mewujudka design and conducted trials and election form The result of design: design of outpatient medical record generated refers to users needs and focus to simplify and facilitate the use of form and quality, a stronger paper.
