PERILAKU UNMETNEED WANITA USIA SUBUR DI KELURAHAN TAMAN KOTA MADIUN

Latar belakang: Unmet need merupakan kelompok wanita usia subur yang saat ini tidak ingin hamil dengan tujuan menjarangkan ataupun membatasi jumlah anak tapi tidak menggunakan kontrasepsi untuk mencegah kehamilannya. Unmet need telah meningkatkan risiko kematian ibu di Indonesia akibat aborsi tidak aman. Tujuan: Penelitian bertujuan untuk mengeksplorasi secara mendalam tentang alasan yang menyebabkan perilaku unmet need ditinjau dari aspek psikologis dan interpersonal. Metode: Penelitian ini merupakan penelitian kualitatif dengan pendekatan fenomenologi. Sampel yang diambil adalah wanita unmet need di wilayah Kelurahan Taman Madiun pada bulan Desember 2013- Februari 2014 dengan teknik purposif sampling. Partisipan dalam penelitian adalah 7 orang wanita unmet need. Analisis data menggunakan content analysis Colaizzi (1978). Hasil: Tema yang ditemukan dari hasil analisis tentang alasan perilaku unmet need antara lain: variasi terhadap pemakaian dan metode kontrasepsi, pengalaman tidak menyenangkan dengan kontrasepsi, persepsi terhadap fertilitas, dukungan suami terhadap perilaku unmet need, kurang pengetahuan tentang kontrasepsi, metode alami pencegah kehamilan, harapan terhadap metode kontrasepsi, dan koping adekuat. Alasan psikologis wanita menjadi unmet need didapatkan dari variasi terhadap pemakaian dan metode kontrasepsi yaitu: ketakutan terhadap efek samping, ketakutan tehadap metode kontrasepsi invasif, pengaruh buruk kontrasepsi terhadap kesehatan dan fertilitas wanita, kesulitan akibat efek samping kontrasepsi, kesulitan dalam menerapkan metode kontrasepsi, dan hambatan internal untuk mendapatkan alat kontrasepsi. Faktor interpersonal adalah dukungan suami terhadap perilaku unmet need dan pelayanan kontrasepsi yang kurang optimal. Kesimpulan: Perilaku unmet need disebabkan faktor psikologis, pengalaman tidak menyenangkan dengan kontrasepsi, kurang pengetahuan tentang kontrasepsi, dan faktor interpersonal.

Introduction: Unmet need is a group of fertile women that are actually do not want to become pregnant currently, in purpose of giving birth spacing or limiting the number of the children but they do not use contraception to prevent the pregnancy. Unmet need has increased the risk of death to mothers in Indonesia because of the unsafe abortion. Objective: This research is aimed to gain the deep information about the reasons causing the unmet need behaviour viewed from psychological and interpersonal aspects. Methods: This research was a qualitative research with a phenomenological approach. The samples were the unmet need women in Taman Village, Madiun in December 2013 - February 2014 with a purposive sampling technique. The participants were seven unmet need women. The data was analyzed using content analysis with Colaizzi’s method. Results: The themes that found from the analysis about the reasons of unmet need behaviour are some varied reasons relate to the contraception use and method, the bad experience in using contraception, perception about the fertility, the husbands’ support to do unmet need behaviour, lack of knowledge about contraception, natural method to prevent of pregnancy, hope of contraception method, and adequate coping. Psychological reasons of unmet need is gained from some varied reasons relate to the contraception use and method; the fear of side effect, the fear of invasive contraceptive method, bad effect of contraception for health and woman fertility, difficulty that caused of side effect contraception, difficulty to apply the contraception method, and internal barrier to get contraception. The interpersonal factors are the husbands’ support to do unmet need behaviour and the minimum quality of contraception service. Conclucion: The unmet need behaviour is caused by the psychological factor, the bad experience in using contraception, lack of knowledge about contraception, and interpersonal factors.
