Latar belakang penelitian : rumah sakit adalah sarana pelayanan kesehatan yang mutlak dibutuhkan oleh segenap lapisan masyarakat dalam upaya peningkatan derajat kesehatan, baik individu maupun masyarakat keseluruhan. oleh karena itu rumah sakit harus meningkatkan kualitas pelayanannya, termasuk diantaranya peningkatan kualitas rekam medis. Tujuan rekam medis adalah untuk menjunjung tertib administrasi dalam rangka upaya peningkatan pelayanan kesehatan di rumah sakit. Lembar resume medis merupakan ringkasan atau inti dari seluruh catatan perawatan atau penatalaksanaan rekam medis terhadap pasien di rumah sakit, dan perannya sangat penting dalam menjamin mutu pelaksanaan medis dirumah sakit terutama dalam hal kelengkapan pendokumentasian rekam medis. Berdasarkan studi awal di RS. Mata “Dr. Yap” peneliti menemukan adanya ketidaklengkapan pada lembar resume medis, sedangkan lembar ini merupakan ringkasan dari seluruh masa perawatan dan pengobatan pasien. Tujuan penelitian : Untuk Mengetahui persentase kelengkapan data identifikasi, laporan penting, dan autentikasi lembar resume, untuk mengetahui faktor-faktor ketidaklengkapan dan upaya untuk meningkatkan kelengkapan lembar resume pada berkas rekam medis rawat inap di RS Mata “Dr. Yap” Yogyakarta. Metode penelitian : jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kualitatif dengan pengumpulan data menggunakan teknik studi dokumentasi, wawancara dan observasi. Rancangan penelitian adalah cross sectional. Untuk validasi data menggunakan triangulasi sumber dan teknik. Pengambilan sampel subyek menggunakan purposive sedangkan obyek penelitian menggunakan perhitungan sampel acak. Penelitian ini dilakukan di Instalasi Rekam Medis RS. Mata “Dr. Yap” Yogyakarta. Hasil: Berdasarkan studi dokumentasi terdapat lembar resume medis pasien JKN lengkap dengan persentase 71,92% dan NON JKN 68,65%, lembar resume medis pasien JKN tidak lengkap dengan persentase 3,26% dan NON JKN 1,53% dan lembar resume medis pasien JKN tidak terisi 24,8% dan NON JKN 29,8% dari masing-masing 40 berkas yang dianalisis. Berdasarkan wawancara dan observasi faktor penyebab ketidaklengkapan terdapat pada sumber daya manusia yaitu dokter, perawat. Sebagian besar dokter yang praktik di RS. Mata “Dr. Yap” adalah dokter tamu, kesibukan dokter dalam melayani pasien, ketidakdisiplinan dokter dalam melengkapi lembar resume medis, keterbatasan waktu praktik, dan dokter bekerja di berbagai pelayanan kesehatan. Sedangkan perawat masih kurang mengingatkan dokter untuk melengkapi lembar resume medis juga menjadi salah satu faktor ketidaklengkapan lembar resume medis. Faktor SDM diatas dalam melakukan pengisian lembar resume medis belum mematuhi peraturan yang ada, sedangkan peraturannya sudah ada dan sudah baik. Selain itu faktor lain yaitu belum diberlakukannya punishment dan reward. Upaya yang dilakukan oleh Instalasi Rekam Medis adalah melakukan verifikasi kelengkapan lembar resume medis, mengembalikan lembar resume medis pada dokter yang bertanggungjawab, melakukan rapat kelengkapan berkas rekam medis dan melakukan komunikasi antara petugas rekam medis dan dokter.
