PERANCANGAN TRACER (OUTGUIDE) UNTUK PENYIMPANAN REKAM MEDIS PERSONAL FOLDER DI PUSKESMAS GONDOKUSUMAN I YOGYAKARTA

Latar Belakang: Pimpinan sarana pelayanan kesehatan bertanggung jawab atas hilang, rusak, pemalsuan, dan/atau penggunaan oleh orang atau badan yang tidak berhak terhadap rekam medis (Permenkes No. 269 / MENKES / PER / III / 2008 tentang kepemilikan, pemanfaatan, dan tanggung jawab Pasal 14). Peneliti menemukan pada penyimpanan berkas rekam medis rawat jalan di Puskesmas Gondokusuman I yaitu, masih belum menggunakan tracer (outguide) untuk rekam medis yang keluar atau dipinjam. Hal ini terjadi dikarenakan sarana prasarana. Dengan tidak adanya tracer (outguide) maka ditemukan kendala-kendala dalam sistem penyimpan diantaranya rekam medis sering terselip atau salah letak dan hilang sehingga menghambat dalam pencarian berkas rekam medis. Menurut International Federation of Health Information Management Associations (IFHIMA, 2012), tracer (outguide) yaitu pengganti rekam medis yang akan dikeluarkan dari penyimpanan untuk tujuan apapun. Harus terbuat dari bahan yang kuat dan berwarna. Ada berbagai jenis tracer yang tersedia. Beberapa termasuk kantong untuk menyimpan permintaan slip dan laporan. Menunjukkan di mana rekam medis ketika tidak ada dalam penyimpanan. Tracer juga meningkatkan efisiensi dan akurasi dengan menunjukkan dimana rekam medis disimpan saat kembali. Menurut Depkes (1997), salah satu ketentuan pokok yang harus ditaati ditempat penyimpanan yaitu tidak satu pun rekam medis boleh keluar dari ruang rekam medis, tanpa tanda keluar/kartu peminjaman. Tujuan: Tujuan dari perancangan ini adalah menghasilkan rancangan tracer (outguide) pada penyimpanan rekam medis personal folder dan membuat draft prosedur penggunaan tracer (outguide) pada penyimpanan rekam medis personal folder di Puskesmas Gondokusuman I Yogyakarta. Hasil: Rancangan tracer (outguide) yang terpilih adalah rancangan alternatif pertama dengan ukuran panjang 34cm dan lebar 15cm, slip permintaan berukuran panjang 10cm dan lebar 7cm, tracer berbahan plastik keras berwarna hijau beserta draft prosedur penggunaannya.

This occurs because of the infrastructure. The absence of tracer in the filing area causing frequent misfile and lost medical record, thus hampering search of medical records. Outguides (or tracers) replace a record that is removed from the files for any purpose. They should be made of strong material, preferably colored. There are various types of outguides available. Some include pockets to store requisition slips and loose reports. Others are cardboard folders, which can also store requisition slips and loose sheets. Others are more sophisticated and expensive. All do the same job; they indicate where the record is when not in file. Outguides (or tracers) also increase filing efficiency and accuracy by indicating where a record is to be filed when returned. When refiling, the requisition slip is removed and available loose reports are filed in the record (IFHIMA, 2012). The cardinal rule in the file area is that no medical record can be removed from file without being replaced by an outguide (Depkes,1997) The purpose of this research is to produce the design of tracer (outguide) and draft procedures of the utilization tracer (outguide) for personal folder medical record filing in Gondokusuman I community health center Yogyakarta. The result of this research is the first alternate design of tracer (outguide) with a length of 34cm and 15cm width was elected and the the requisition slip with a length of 10cm and 7cm width. Tracer made of hard plastic and the color is green with draft prosedures of the utilization tracer (outguide).
