LEPTIN GENE POLYMORPHISM IN ISCHEMIC STROKE PATIENTS WITH INSULIN RESISTANCE GRADUATING PAPER

BACKGROUND: Ischemic stroke is a heterogeneous disease with various underlying pathophysiology. Genetic factor plays a crucial role among non-modifiable risk factors. OBJECTIVE: This research aims to identify leptin gene polymorphism as a risk factor for insulin resistance and ischemic stroke. METHOD: This research consists of a total of 84 samples, 42 cases and 42 controls. Case refers to blood sample of ischemic stroke patient while control refers to blood samples of non-ischemic stroke patient with no history of stroke. Blood samples of all subjects underwent DNA isolation and Polymerase Chain Reaction (PCR) and electrophoresis were done and visualized by staining with ethidium bromide and viewed under ultraviolet (UV) rays to study the polymorphism of microsatellite allele of Leptin gene in these samples. RESULTS: Results showed that there is no direct correlation between leptin gene polymorphism, incidence of ischemic stroke and insulin resistance. Correlation of leptin genotype with ischemic stroke (OR=0.459; 95%CI=0.191-1.102) is insignificant (P=0.062) and correlation of leptin genotype with insulin resistance (OR=0.795; 95%CI=0.252-2.508) is also insignificant (P=0.459). Class I/II leptin genotype is most common among Indonesian population. Further observation on other contributing factors shown that fasting blood glucose (P=0.001), HOMA-IR (P=0.027) and diabetes mellitus (P=0.015) were significant risk marker for ischemic stroke and will increase the incidence of ischemic stroke by 37%, 24.2% and 26.6% respectively. Besides that, fasting blood insulin (P=0.011) remained as significant risk marker for insulin resistance and will increase the incidence by 27.7%. CONCLUSION: Leptin genotype does not directly associate with increased risk for ischemic stroke and insulin resistance. Class I/II leptin genotype is the most common genotype among population in Banda Aceh, Indonesia.
