HUBUNGAN KADAR IL 8 SERUM, JUMLAH LEUKOSIT DAN NEUTROFIL DENGAN KEJADIAN ARDS PASCA OPERASI MAYOR DI RSUP Dr. SARDJITO

Trauma pembedahan menyebabkan perubahan hemodinamik, metabolisme, dan respon imun pada periode pasca operasi. 1 Disfungsi imun dapat menyebabkan kegagalan organ pada pasien trauma berat. 2 Lung Injury adalah istilah yang digunakan untuk menggambarkan respon paru terhadap berbagai trauma langsung yang terjadi di paru-paru atau sebagai konsekuensi dari trauma atau peradangan di lain. Proses inflamasi sebagian dikendalikan oleh sitokin termasuk TNF-α, IL-1β, IL-6, dan IL-8, yang diproduksi oleh sel inflamasi dan dapat mempromosikan adhesi neutrofil-endotel, kebocoran mikrovaskular, dan memperkuat respon proinflamasi lain. 1 Pada ARDS terjadi injuri parenkim paru yang difus dan akut sebagai akibat dari peningkatan permeabilitas, udem paru, hipoksemia berat dan disfungsi mekanik. Acut respiratory distress syndrome memberikan kontribusi morbiditas dan mortalitas pada pasien yang dirawat di ICU di seluruh dunia dan berakibat kerugian material dan nonmaterial yang berat. 4 Adanya biomarker yang dapat memperkirakan terjadinya ARDS dan progresifitasnya akan sangat bermanfaat. 6 Penelitian ini adalah penelitian deskriptif analitik dengan rancangan prospektif observasional. Pasien yang menjalani operasi mayor antara Oktober sampai dengan Desember 2011, dilakukan informed consent, pemeriksaan klinis laboratoris. Pada hari ke-3 post operasi dilakukan pemeriksaan kadar IL-serum, jumlah leukosit, jumlah neutrofil dan AGD. Tujuan penelitian ini adalah untuk mengetahui apakah terdapat hubungan antara kadar IL-8 serum, jumlah leukosit dan jumlah neutrofil dengan kejadian ARDS pada pasien pasca operasi mayor. Sebanyak 32 pasien memenuhi kriteria inklusi, terdiri dari 15 orang lakilaki dan 17 perempuan. Terdapat 1 kasus ARDS dengan kadar IL-8 71.38 pg/ml, paling tinggi dibandingkan dengan kasus non ARDS. IL-8 berpotensi menjadi prediktor kejadian ARDS. Terdapat hubungan yang bermakan antara jumlah leukosit dan jumlah neutrofil terhadap rasio PaO2/FiO2.  Jumlah neutrofil dapat dijadikan faktor predisposisi independen terhadap rasio PaO2/FiO2, dapat mempengaruhi sebesar 19 %.

Surgical trauma prosedures alterations in the hemodynamic, metabolic, immune responses of patients in the postoperative period. 1 Immune dysfunction can provoke (multiple) organ failure in severely injured patients. 2 injury is the term used to describe the pulmonary response to a broad range injuries occurring either directly to the lung or as the consequence of injury or inflammation at other sites in the body. The inflammatory process is driven in part cytokines including TNF-α and IL-1β, IL-6, and IL-8. 1 They are produced by inflammatory cells and can promote neutrophil-endothelial adhesion, microvascular leakage, and amplify other proinflammatory responses. Acut respiratory distress syndrome continues as a contributor the morbidity and mortality of patients in intensive care units (ICU) throughout the world, imparting tremendous human and financial costs. 4 Identifying a biomarker that predicts the development of ARDS or progression of severity could be helpful. 6 This study was a descriptive analysis with an observational prospective design. Patients who underwent major surgeries during October until December 2011, were informed consent and had their clinical condition leucocyte count, neutrophil count and blood gas analysis checked before surgery. On the third day surgery, the patients had their blood gas analysis, serum IL-8, leucocyte and neutrophil count checked. The purpose of this study was to understand the correlation between IL-8 serum level, leucocyte and neutrophil count with incidence of ARDS. There were 32 patients included in the criteria, which consisted of 15 males and 17 females. We found 1 cases of ARDS, with IL-8 serum level was 71.38 pg/ml, highest level than non ARDS.  IL-8 was a potential predictor of ARDS. There was a correlation between leucocyte and neutrophil count with FiO2 ratio. Neutrophil was an independence predictor for PaO2/FiO2 ratio 19%).
