ANALISIS BIAYA TERAPI PASIEN DIABETES MELITUS RAWAT INAP RSUD Dr. MOEWARDI SURAKARTA PERIODE TAHUN 2010

Di Indonesia, diabetes melitus merupakan penyakit kronis yang banyak diderita dan membutuhkan banyak biaya, maka analisis biaya menjadi hal yang penting untuk dipertimbangkan. Tujuan penelitian ini untuk mengetahui komponen biaya, rata-rata biaya keseluruhan, dan mengetahui apakah terdapat korelasi antara faktor pasien dan jenis obat terhadap biaya total, antara jenis pembiayaan dan kelas perawatan dengan biaya medis langsung pada pasien diabetes melitus rawat inap di RSUD Dr. Moewardi Surakarta. Penelitian analisis biaya pasien diabetes melitus rawat inap RSUD Dr. Moewardi Surakarta menggunakan rancangan penelitian deskriptif non eksperimental, data diambil secara retrospektif dari catatan medik pasien dengan diagnosis utama diabetes melitus dengan penyakit penyerta yang memenuhi kriteria inklusi selama periode Januari sampai dengan Desember 2010, bagian keuangan RSUD Dr. Moewardi Surakarta untuk mengetahui biaya serta dari bagian instalasi farmasi untuk mengetahui rincian penggunaan obat oleh pasien selama satu periode pengobatan. Pengambilan sampel menggunakan teknik simple random sampling. Uji chi square digunakan untuk mengetahui hubungan antara faktor pasien, jenis obat, jenis pembiayaan, dan kelas perawatan terhadap biaya total. Hasil penelitian yaitu komponen biaya yang menyusun keseluruhan biaya pengobatan pasien diabetes melitus rawat inap RSUD Dr. Moewardi Surakarta periode tahun 2010 adalah biaya medis langsung yang terdiri dari biaya obat, biaya administrasi, biaya IGD, biaya laboratorium, biaya rawat inap, biaya tindakan, biaya visite, biaya Alat Medis Habis Pakai, dan Bahan Medis Habis Pakai. Rata-rata biaya pengobatan total pada pasien JAMKESMAS+PKMS+tidak mampu adalah Rp 4.127.180 dan pada pasien ASKES PNS+Umum+Kerjasama adalah Rp 3.828.282. Komponen biaya terbesar pada pasien JAMKESMAS+PKMS+tidak mampu adalah biaya laborat (27,02%), biaya obat (25,74%), dan biaya tindakan (17,20%), sedangkan pada pasien ASKES PNS+Umum+Kerjasama komponen biaya terbesar adalah biaya obat (27,54%), biaya laborat (23,02%), dan biaya tindakan (19,00%). Hasil uji chi square menunjukkan ada hubungan antara lama waktu perawatan (LOS) dengan biaya pengobatan diabetes melitus di RSUD Dr. Moewardi Surakarta.

In Indonesia, diabetes melitus is a chronic disease which is suffered and need much cost, therefore the cost analysis become an important thing to be considered. The aim of this research is to know cost component, whole mean cost, and to know whether there is correlation between patient factor and types of medicine trough total cost, between types of fund and treatment class with direct medical cost on diabetes melitus in patient RSUD Dr. Moewardi Surakarta. Research on cost analysis diabetes melitus hospitalizing stay patient in RSUD Dr. Moewardi Surakarta used descriptive non experimental research design, data has been taken restrictively from patients medical record with main diagnose diabetes melitus with comorbid which meets inquiry inclusion during periode of January to December 2010, financial department of RSUD Dr. Moewardi Surakarta to know the cost needed and from the pharmacy installation department to know the details of medical from usage by patient during one treatment period. The samples were taken by using simple random sampling. Chi square test was used to know the relation between patient factor, types of medicine, types of fund, and treatment class to the total cost. The result shows the cost which arrange on diabetes melitus in patient RSUD Dr. Moewardi Surakarta periode of 2010 total cost are medical direct cost such as medical cost, administration cost, IGD cost, laboratory cost, hospitalizing stay cost, action cost, visit cost, consumption medical tools, consumption medical source. The mean of treatment total cost on “JAMKESMAS+PKMS+tidak mampu” patients are Rp.4,127,180 and on “ASKES PNS+Umum+Kerjasama” patients are Rp. 3,828,282. the biggest cost on “JAMKESMAS+PKMS+tidak mampu” patients are laboratory cost (27.02%), medical cost (25.74%) and action cost (17.20%), while the biggest component on “ASKES PNS+Umum+Kerjasama” patients are medical cost (27.54%), laboratory cost (23.02%), and action cost (19.00%). The result of chi square test shows that there is relation between the Length Of Stay (LOS) and treatment cost of diabetes melitus in RSUD Dr.Moewardi Surakarta.
