Analisis hambatan pelaksanaan standar pelayanan farmasi di RSUD Klas C Jawa tengah

InstaIasi Farmasi sebagai salah satu pelayanan di rumah sakit dituntut untuk selalu mengupayakan pemngkatan mutu p e l a y m Upaya pemgkatan mutu pelayanan farmasi dengan menerapkan standar pelayanan farmasi yang telah ditetapkan oleh Departemen Kesehatan. Berdasarkan survey pendahuluaa pelaksanaan standar pelayanan farmasi yang dinilai berdasarkan insfrument self msessntenf skor yang dicapai masih rendah dan banyak mengalarm hambatan, untuk itu perlu dketahui dan dievahmi bagaixnana pelaksanaan standar pelayauan farmasi di Rumah Sakit Klas C Jawa Tagah, sehmgga pelayanan fbmasi dapat optimal. Penelitian dilalnrkan dengan cara observasi non experimental dengan pemilii lokasi Rumah Sakit Umum Daerah Klas C di Jawa Tengah dengan memilih secara acak tiap karesidenan diambil satu. Dalam permlrhan didapat enam bmah Sakit Umum Daerah yang memenuhi kriteria yaitu Rumah Sakit Umum milik Pemerintah Daerah Klas C, sudah mempunyai Apoteker dan sudah terakreditasi lima bidang pelayanan. Dalam penelitian hi menggunakan data primer yang didapat dari peniiaian pencapaian self assessment dan wawancara dengan Kepala Instalasi Farmasi dan Direktur rumah sakit. Alat ukur yang digunakan lembar catatan untuk penilaii kuisioner dan alat perekam untuk wawancara. Hasil penelitian didapatkan bahwa pelaksanaan standar pelayanan farmasi dicapai skor bervariasi dari 34,27%-52,85%. Hambatan yang dialami dalam pelaksanaan standar pertama adalah: kurangnya pemahaman tentang falsafhh dan tujuan, belum ada sosialisasi dari pihak manajemen, konsentrasi terpusat pada pelayanan, belum melaksanakan pelayanan f m i kliikstandar kedua adalah: kurangnya tenaga, dan kegiatan PFT belum bejalan dengan baik, standar ketiga adalah: tenaga kefannasian masih kurang, standar keempat adalah : fasilitas, sarana dan prasarana kurang memadai karena kekurangan dana, kelima adalah : kurangnya tenaga dan beban pekerjaan terlalu banyak, belum ada kemauan dari stafl hngsi PFT belum berjalan, pelaksanaan kebijakan dan prosedur belum berjalan dengan bak, standar keenam adalah : beban pekerjaan terlalu hauyak, belum ada kewajiban untuk membuat program orientasi bagi pegawai baru dari pihak manajemen, organisasi belum kearah pengembangan dan pendidikan staf dan tidak tersedia dana, standar ketujuh adalah: tenaga kurang, tidak ada kemauan dari staf, prosedur evaluasi belum baku, organisasi dan tenaganya belum tdentuk. Dari ketujuh standar pelayanan farmas4 faktor utama yang menjadi hambatan datam pelaksanaan standar pelayanan farmasi adalah kurangnya tenaga farmasi di Instalasi Farmasi

Pharmacy dept as one of hospital service is demanded to improve its service quality continually. One &ort to improve the pharmacy service quality is c~nducted by applying the standard pharmacy services that have been standardized by the Dept of Hdth, Republic of Indonesia. Based on 811 introductory survey on standard pharmacy service implementations which is measured accord= to self-assessment instrument, it is found that the score of the hospital pharmacy seMces is tow. It is also found that the hospital pharmacy Senices have faced a lot of obstacles. Therefore, in order to be able to optimize the pharmacy service qualify, the implementations of standard pharmacy service in C-clas hospital in central Java have to be evaluated This research uses the ohservationd non-experimental design/ method.The research is carried on in C-class government hospital in central java, in every residency one hospital is chosen/ selected randomly. The writer then finds that 6 regionat government hospitals have met the criteria, namely general hospitals which belong to C-class district government, they have already had pharmacists,and they have already been accredited in five service fields. The primary data are obtained fiom observations with the appraisal of self-assessment achievement and interviews with the head of pharmacy depts and the hospital directors. The instruments used werw note sheets in the observations and questionnaires and tape recorders in the inteviews. The result of this research shows that the implementations of standard pharmacy sevice reach varied score ranging fiom 34,27?! - 52,85%. The obstacles faced in the first standard implementation are the lack of understanding about the philosophy and the purposes, there is no socialization process the manager, the concentration is focused on services, and the clinical pharmaceutical service is not implemented yet. In the second standard implementation, there are lack of human resources, and the activity of the PFT does not work well. Ln the third standard implementation, there is lack of human resources in the pharmacy. There is lack of facility and infiastructures because of limited finding in the fourth standard implementation. The obstacle that faced in the fifih standard implementation are the lack of human resources and there are too many job burdens, staff motivation is low, PFT fbnction does not work well, and the implementation of the policy and procedure does not work yet. In the sixth standard implementation, there is too may job burdens and there is no obligation from the management &F to make orientation program for new employee, the organization and the stafFeducation are not yet developed and there is no avaible h d . Fiaally the obstacle that is found in the seventh stapdard implementation are the lack of human resources, there no staff motivation, the evahion procedure is not formabed, and the organization and the human resources are not formed yet. Indeed out of there seventh standard implementations, the main factor that become a serious obstacle in the implementation of the standard pharmaceutical service is the Iack of the human resources in the pharmacy dept.
