Latar Belakang: Pengumpulan data sensus harian di rumah sakit merupakan data yang harus dikumpulkan setiap hari. Data tersebut berguna untuk memantau perawatan pasien setiap hari, minggu, bulan. Selanjutnya data yang terdapat pada sensus harian tersebut akan diolah dalam satu rekapitulasi dan akan menghasilkan pelaporan. Sensus berguna untuk mengetahui jumlah pasien masuk, pasien keluar, dan pasien meninggal. Selain itu untuk mengetahui tingkat penggunaan tempat tidur dan menghitung penyediaan fasilitas pelayanan kesehatan.Oleh karena itu sensus harian pasien rawat inap harus dibuat seakurat mungkin guna menunjang pelaporan rumah sakit yang cermat, tepat, serta akurat. Tujuan : Tujuan umum dari penelitian ini adalah mengetahui faktor penyebab ketidakakuratan sensus harian pasien rawat inap di Rumah Sakit Jiwa Grhasia DIY. Tujuan khusus dari penelitian ini adalah mengetahui proses pembuatan sensus harian pasien rawat inap di bangsal perawatan dan melakukan analisa penyebab ketidakakuratan sensus harian pasien rawat inap. Metode Penelitian : Penelitian ini menggunakan metode deskriptif dengan pendekatan kualitatif. Subyek dalam penelitian ini adalah petugas rekam medis bagian pelaporan, kepala rekam medis dan perawat bangsal. Obyek dalam penelitian ni adalah sensus harian pasien rawat inap. Hasil : Pelaksanaan sensus harian pasien rawat inap masih berjalan secara manual dan sedang dalam tahap proses perubahan ke elektronik. Proses pembuatan sensus harian rawat inap dilakukan pada malam hari oleh petugas bangsal, kemudian pengambilan dilakukan oleh petugas instalasi rekam medik pada pagi harinya. Dalam pelaksanaanya masih ditemukan ketidakakuratan dalam sensus harian pasien rawat inap. Terdapat beberapa faktor yang menyebabkan ketidakakuratan dalam pelaksanaan sensus harian pasien rawat inap. Faktor tersebut antara lain SDM dan koordinasi antar petugas rekam medis dengan petugas bangsal.

Objective: The general objective of this study was to determine the factors that cause inaccuracies daily census of inpatients in mental hospital Grhasia DIY. The specific objective of this study was to determine the process of making daily census of inpatients in the ward and to analyze the causes of inaccuracies daily census of inpatients. Methods: This study used a descriptive method with qualitative approach. The subjects in this study were part of the medical record reporting officer, chief medical records and ward nurses. Ni is the study of objects in daily census of inpatients. Results: Implementation daily census of inpatients is still running it manually and are under the process of changing to electronic. The process of making daily inpatient census carried out at night by the ward clerk, then decision made by the installation of officers at the medical records of the morning. In practice still found inaccuracies in the daily census of inpatients. There are several factors that lead to inaccuracies in the implementation of the daily census of inpatients. These factors, among others, human resources and coordination between medical records clerk with the ward officer.
