Kelengkapan Pengisian Lembar Resume Dokter Terkait Persiapan Akreditasi KARS 2012 di Rumah Sakit Jiwa Grhasia DIY

Latar Belakang: Dilihat dari evaluasi sasaran mutu pada bulan Juli-Desember 2012, prosentase ketidaklengkapan dokumen rekam medis sebesar 5,827%. Ketidaklengkapan tersebut dapat dijabarkan menjadi 0,03% untuk data sosial lembar resume dokter, 2,093% untuk penulisan bukti rekaman pada lembar resume dokter, 3,698% untuk tanda bukti keabsahan pada lembar resume dokter, dan 0,005% tatacara pencatatan pada lembar ringkasan masuk dan keluar. Ketidakterisian lembar resume dokter dapat mempengaruhi pelayanan, klaim asuransi, dan dapat menyebabkan penumpukan berkas rekam medis sehingga dapat mempengaruhi pekerjaan petugas rekam medis. Mengingat pentingnya resume dokter, banyaknya dampak yang ditimbulkan dari ketidakterisian tersebut dan adanya poin-poin dalam penilaian akreditasi yang berhubungan dengan resume medis Tujuan: Tujuan penelitian ini adalah untuk mengetahui pengisian lembar resume dokter dalam pemenuhan standar akreditasi, mengetahui penyebab ketidakterisian lembar resume dokter dan upaya yang dilakukan petugas rekam medis dan profesi terkait dalam mengatasi ketidakterisian lembar resume dokter. Metode Penelitian: Penelitian ini menggunakan deskriptif pendekatan kualitatif, dengan rancangan penelitian cross-sectional. Teknik pengumpulan data menggunakan wawancara, observasi, dan studi dokumentasi. Untuk validitas data menggunakan triangulasi teknik. Hasil: Pengisian lembar resume dokter dalam pemenuhan standar akreditasi adalah lembar resume diisi oleh DPJP setelah pasien pulang, lembar resume dokter di Rumah Sakit Jiwa Grhasia DIY sudah terdapat instruksi untuk tindak lanjut, salinan ringkasan pelayanan pasien didokumentasikan dalam rekam medis dalam lembar resume dokter, salinan resume dokter diberikan pada pasien jika diminta oleh keluarga pasien, salinan resume dokter diberikan pada praktisi kesehatan perujuk dengan menyalin pada lembar balasan rujukan, di Rumah Sakit Jiwa Grhasia DIY belum ada kebijakan yang menetapkan kapan resume pasien pulang harus dilengkapi dan dimasukkan ke rekam medis pasien, lembar resume dokter di Rumah Sakit Jiwa Grhasia DIY di dalamnya sudah mencakup alasan pasien dirawat, diagnosis dan penyakit penyertanya, temuan fisik dan hal lain yang penting, prosedur diagnostik dan terapetik yang telah dilakukan, medika mentosa dan obat waktu pulang, keadaan/status pasien saat pulang, dan instruksi untuk tindak lanjut. Untuk prosentase kelengkapan pengisian lembar resume dokter adalah 57% untuk komponen identitas pasien, 24%untuk komponen bukti rekaman,3% untuk komponen keabsahan rekaman, dan 16% untuk tatacara pencatatan. Penyebab ketidakterisian lembar resume dokter adalah jumlah dokter dan kesibukan dokter, kurangnya sosialisasi terkait pengisian lembar resume dokter, dan tidak adanya reward and punishment. Upaya yang dilakukan dalam mengatasi ketidakterisian lembar resume dokter adalah petugas rekam medis meng-assembling dan melakukan analisis kuantitatif dan diberi memo setelah itu diantarkan ke ruang dokter. Dokter berusaha melengkapi secepatnya. Jika ada dokter yang libur/cuti pengisian dapat di isikan dengan mengatasnamakan dokter yang bertanggung jawab. Komite rekam medik melakukan teguran pada dokter yang bersangkutan dan melakukan evaluasi.

Purpose: The purpose of this study was to determine the charging doctor’s resume sheet in fulfillment accreditation standards, determine the cause incompleteness doctor’s resume sheet and efforts of medical records personnel and related professions overcome of incompleteness doctor’s resume sheet. Methods: This study used a qualitative descriptive approach, with a cross-sectional study design. Techniques of data collection using interviews, observation, and documentation. For the validity of the data using triangulation techniques. Result: Charge doctor’s resume sheet in fulfillment accreditation standards are filled out by DPJP after the patient goes home, doctor’s resume sheet at Grhasia Psyciatric Hospital DIY have instructions for follow-up, a copy of the summary care patients documented in the medical record in the doctor’s resume sheet, a copy of doctor’s resume sheet given to patients if requested by the patient's family, doctor’s resume sheet copy given to the referring medical practitioner with a copy on reply sheet references, in the Grhasia Psyciatric Hospital DIY no policy that specifies when the patient resumes return must be completed and entered into the patient's medical record, doctor’s resume sheet at Grhasia Psyciatric Hospital DIY in it already include the reasons patients are treated, diagnosis and concomitant diseases, physical findings and other things that are important, diagnostic and therapeutic procedures that have been carried out, and the medicinal mentosa medika time home, state / status of current patients home, and instructions for follow-up. For completeness percentage resume charging sheet physicians was 57% for patients identity component, 24% for component recording of evidence, 3% for the validity of the recording component, and 16% for the recording procedure. Cause incompleteness resume sheet is the number of physicians and rushing doctors, lack of socialization related charging of doctor’s resume sheet, and the absence of reward and punishment. Efforts made in overcome of incompleteness doctor’s resume sheet are assembling medical records and perform quantitative analysis and given the memo. After it the medical record was delivered to the doctor's office. Doctors try to complete as soon as possible. If there is a doctor who holiday / leave charging can fill in the name of the doctor in charge. Committee on the medical record did reprimand the doctor concerned and evaluation.
