ANALISIS KETEPATAN KODE DIAGNOSIS DAN TINDAKAN KASUS PERSALINAN DENGAN PENYULIT PASIEN JAMPERSAL DI RSUP DR. SOERADJI TIRTONEGORO KLATEN

Latar Belakang : cara penyimpanan berkas rekam medis RSUP Dr. Soeradji Tirtonegoro Klaten adalah desentralisasi dimana rekam medis rawat jalan dan rekam medis rawat inapnya dipisah. Sehingga catatan riwayat kesehatan pasien tidak tersusun secara kronologis. Tujuan Penelitian : memberikan gambaran mengenai proses pengodean serta prosentase ketepatan pengodean diagnosis kasus persalinan dengan penyulit serta prosentase kesesuaian baik diagnosis maupun kode diagnosis di RSUP Dr. Soeradji Tirtonegoro Klaten. Metodologi Penelitian : penelitian ini menggunakan metode penelitian deskriptif dengan pendekatan kualitatif. Rancangan penelitian ini menggunakan pendekatan fenomenologi. Subjek pada penelitian ini adalah petugas coding rawat inap, sedangkan objek penelitian ini adalah berkas rekam medis rawat inap persalinan dengan penyulit. Teknik pengambilan data : observasi, wawancara dan studi dokumentasi. Hasil Penelitian : Kesesuaian diagnosis antara rawat jalan dengan rawat inap mencapai 100%; pelaksanaan pengodean di RSUP Dr. Soeradji Tirtonegoro Klaten sudah sesuai dengan SPO yang ada dengan menggunakan ICD-10, ICD-9CM serta “buku pintar”; tingkat ketepatan kode untuk rawat jalan mencapai 58,06%, untuk rawat inap yaitu tindakan 94,23%, metode melahirkan 86,36%, penyulit 62,71%serta outcome delivery 11,36%; prosentase kesesuaian kode antara rekam medis rawat inap dengan lembar INA-CBG’s mencapai 77,60%untuk tindakan, 70,45%untuk metode melahirkan dan 51,72%untuk penyulit.

Background: The file storage method of medical record in RSUP Dr. Soeradji Tirtonegoro Klaten is decentralized where outpatient medical records and inpatient medical records are separated. So, the patient's medical history is not arranged in chronological order. Purpose: provide an overview of the process of coding; provide an overview about accuracy code of diagnose percentage for labor with complication cases; provide an overview about precentage of the suitability of the outpatient’s diagnose and inpatient’s diagnose; provide an overview about the percentage of the suitability of diagnose’s codes in RSUP Dr. Soeradji Tirtonegoro Klaten. Method: This study uses descriptive method with qualitative approach. The design of this study used a phenomenological approach. Subjects in this study were inpatient coding personnel, while the object of this study is the inpatient medical record for labor with complication cases. Techniques of data collection: observation, interview and documentation. Results: Compliance with an outpatient diagnosis between inpatient reach 100%; implementation of coding in Dr. Soeradji Tirtonegoro Klaten is in conformity with the existing SPO using ICD-10, ICD-9CM and \\"book smart\\"; accuracy rate for outpatient code reaches 58.06%, which is the treatment/procedure 94.23%, delivery method 86, 36%, complication and outcome delivery 62.71% and 11.36%; percentage of code compatibility between the inpatient medical record and INA-CBG's reached 77.60% for treatment/procedure, 70.45% for delivery method and 51.72% for complication.
