ANALISIS BIAYA TERAPI SECTIO CAESAREA SEBAGAI PERTIMBANGAN DALAM PENETAPAN PEMBIAYAAN KESEHATAN BERDASARKAN INA-DRGs di RSUP Dr. MOHAMMAD HOSEIN PALEMBANG

Implementasi sistem INA-DRG bagi pasien kelas III Jamkesmas di rumah sakit diharapkan mampu meningkatkan askes dan mutu pelayanan kesehatan kepada seluruh masyarakat miskin dan tidak mampu dalam rangka mewujudkan tercapainya pelayanan kesehatan di rumah sakit yang optimal secara efektif dan efisien. Tujuan penelitian ini adalah memperoleh gambaran rata-rata biaya pengobatan pada Sectio caesarea di rumah sakit, mengetahui faktor yang berpengaruh terhadap biaya perawatan serta untuk mengetahui kesesuaian antara biaya riil pengobatan sectio caesarea di RSUP Dr. Moh. Hosein dengan pembiayaan kesehatan berdasarkan INA- DRGs. Jenis penelitian adalah deskriptif menurut perspektif rumah sakit. Metode pengambilan data dilakukan secara retrospektif yang diambil dari penelusuran dokumen catatan medik pasien bedah sectio caesarea dan data biaya pengobatan pasien. Subjek penelitian adalah pasien bedah sectio caesarea kelas 3 peserta Jamkesmas di RSUP Dr. Moh. Hosein periode Januari-Desember 2010. Analisis data dilakukan secara crosstab dan linier regression serta perbandingan rata-rata biaya dan LOS dengan tetapan INA-DRG. Nilai p dari hasil uji Chi Square 102 pasien Sectio caesarea antara umur, jumlah paritas, indikasi medis, tingkat keparahan dan LOS terhadap biaya perawatan adalah 0,091, 0,634, 0,816, 0,408 dan 0,300. Rata-rata biaya terapi Sectio caesarea untuk pasien Jamkesmas berdasarkan tarif RSUP Dr. Moh. Hosein pada tingkat keparahan I sebesar Rp. 8.022.374,- ± Rp. 719.474,3 dengan ALOS riil 5,19 hari, sedangkan pada tingkat keparahan II sebesar Rp. 8.297.799,- ± Rp. 845.418,7 dengan ALOS riil 5,07 hari dan pada tingkat keparahan III sebesar Rp. 7.957.528,- ± Rp. 600.405,3 dengan ALOS riil 5,50 hari. Terdapat perbedaan yang signifikan antara biaya riil RS dengan tetapan INA-DRG, sehingga besar rata-rata biaya pengobatan Sectio caesarea berdasarkan tarif di RSUP Dr. Moh. Hosein belum sesuai dibandingkan dengan tetapan INA-DRG.

The data sampling is done retrospectively by investigating the medical record and data of medical of surgical sectio caesarea patient. The subject of this reserach is the third class surgical sectio caesarea patient and the member of Jamkesmas program in RSUP Dr. Moh Hosein during the period of January-December 2010. The data analysis is done through crosstab and linier regression and the comparison of average medical care cost and LOS is based on the standard of INA-DRG. sectio caesarea The value of p from the experiment result Chi Square of 102 patient between age, parity number, medical indication, severity level and LOS towards the medical care cost are 0,091, 0,634, 0,816, 0,408 and 0,300. The average therapy cost of sectio caesarea for Jamkesmas patient based on the tariff of RSUP Dr. Moh. Hosein at severity level I IDR 8.022.374,- is ± IDR 719.474,3 with the real ALOS 5,19 days, while at severity level II IDR 8.297.799,- is ±IDR 845.418,7 with real ALOS 5,07 days, and at severity level III IDR 7.957.528,- is ± IDR 600.405,3 with real ALOS 5,50 days. There is a significant difference between the real hospital cost and the standard of INA-DRG so that it can be concluded that the medical care average cost of Sectio caesarea based on the RSUP Dr. Moh. Hosein tariff is not compatible with the tariff standard of INA-DRG.
