PERBEDAAN PEMAKAIAN PROTESA OBTURATOR RESIN AKRILIK DAN THERMOPLASTIC NYLON TERHADAP HASIL KUNYAH PADA PENDERITA KLAS II ARAMANY PASCA HEMIMAXILLECTOMY

Defek maksila pasca hemimaxillectomy mengakibatkan menurunnya fungsi pengunyahan, bicara, penelanan, dan cacat wajah. Menurunnya fungsi mempengaruhi penampilan dan psikologis penderita. Salah satu perawatannya menggunakan alat rehabilitasi protesa obturator resin akrilik atau thermoplastic nylon. Tujuan penelitian ini untuk mengetahui perbedaan pemakaian protesa obturator resin akrilik dan thermoplastic nylon terhadap hasil kunyah pada penderita kelas II pasca hemimaxillectomy. Penelitian ini menggunakan 6 subjek pasca hemimaxillectomy. Subjek memakai protesa obturator resin akrilik diinstruksikan mengunyah 10g kacang tanah dengan 20 kunyahan, hasil kunyahan dikeluarkan dalam gelas ukur dengan kumur air bersih. Hasil kunyah dalam gelas ukur dituangkan di atas tumpukan mesh 24, 12 dan ayakan tepung. Partikel setiap ayakan ditempatkan pada kertas saring dan cawan petri, keringkan dalam inkubator 60°C selama 24 jam, ditimbang dan dicatat. Pemeriksaan tersebut diulangi dengan memakai protesa obturator thermoplastic nylon. Data dianalisis dengan independent uji t dengan tingkat kepercayaan 95% (α=0,05). Hasil penelitian menunjukkan terdapat perbedaan yang signifikan pada penderita dengan memakai protesa obturator resin akrilik dan thermoplastic nylon terhadap hasil kunyah pasca hemimaxillectomy. Kesimpulan penelitian ini adalah: 1) terdapat perbedaan pemakaian protesa obturator resin akrilik dan thermoplastic nylon terhadap hasil kunyah pada penderita kelas II Aramany pasca hemimaxillectomy; 2) pemakaian protesa obturator thermoplastic nylon meningkatkan hasil kunyah lebih halus daripada protesa obturator resin akrilik pada penderita kelas II Aramany pasca hemimaxillectomy.

Maxilla defect after hemimaxillectomy result in decrease of mastication, speech, and swallowing function and also facial defects. Functional reduction affects the patient’s appearance and psychology. The treatment of this condition is by using rehabilitation appliance with acrylic resin or thermoplastic nylon obturator prosthesis. The purpose of this study was to determine the difference of obturator prosthesis using acrylic resin and nylon thermoplastic on chewing result in patients class II Aramany pasca hemimaxillectomy. This study used six pasca hemimaxillectomy patients. Subjects wearing obturator prostheses were instructed to chew 10g peanuts with 20 times chew and the bolus is threw out to a measuring cup with clean water. The bolus was poured above 24 and 12 mesh stacks and a flour sieve. Particles sieve in filter paper and placed petri dish and dehydrated in 60 ° C incubator for 24 hours and then weighed and recorded. Examination was repeated by using thermoplastic nylon obturator prosthesis. Data were analyzed with independent t-test 95% confidence level (α = 0,05). The results showed significant differences in patients with acrylic resin and nylon thermoplastic obturator prostheses to mastication function pasca hemimaxillectomy. This study conclude: 1) there were differences in used obturator prostheses acrilic resin and nylon thermoplastic of the chewing result in patients with class II Aramany pasca hemimaxillectomy; 2) nylon thermoplastic obturator prosteses could improve more subtle chewing result in patients with class II Aramany pasca hemimaxillectomy.
