RESPONSIVENESS SKOR CONSTANT-MURLEY’S SHOULDER SEBAGAI ALAT UKUR HASIL AKHIR FRAKTUR KLAVIKULA SEPERTIGA TENGAH

Latar belakang : Constant-Murley’s Shoulder merupakan metode valid pengukuran status kesehatan yang berguna pada pasien-pasien dengan variasi keluhan-keluhan yang beragam pada ekstremitas superior.Alat ukur ini cukup sensitive untuk mendteksi setiap perubahan-perubahan kecil dalam hal fungsi.Metode alat ukur ini sangat mudah dipergunakan dan membutuhkan waktu yang relative singkat untuk mengevaluasi suatu kelompok populasi yang besar. Obyektif : Penelitian ini bertujuan mengevaluasi responsiveness hasil akhir pengukuran instrument Constant-Murley’s Shoulder pada fraktur klavikula sepertiga tengah. Metode : Jumlah populasi penelitian adalah 200 pasien dengan diagnosis fraktur klavikula sepertiga tengah berbasis criteria pada pemeriksaan klinis dan radiologis.Rancangan klinis menggunakan. penelitian Classic Cohort dengan jangka pengukuran 3 bulan dan 6 bulan paska tindakan konservatif ataupun operatif.Responsiveness dievaluasi berdasarkan nilai perhitungan effect size (ES) dan standardrized response mean (SRM). Hasil Penelitian : Instrumen Constant-Murley’s Shoulder menunjukan hasil yang sangat responsive (effect size (ES): 1,38 dan standardrized response mean (SRM): 1,51). Pembahasan : Instrumen effect size (ES) dan standardrized response mean (SRM sangat sensitive dalam mendeteksi setiap perubahan klinis yang berguna mengevaluasi hasil akhir penanganan fraktur klavikula sepertiga tengah paska tindakan konservatif dan operatif pada bulan ke-3 dan ke-6.

Background: Constant-Murley’s Shoulder is a valid measure of health status useful in patients with a wide variety of upper-extremity complaints, it is sufficiently sensitive to reveal even small changes in function. The method is easy to perform and requires a minimal amount of time for evaluation of large population groups. Objective: To evaluate the responsiveness Constant-Murley’s Shoulder instrument as outcome measure for midshaft clavicle fracture. Method: The study population was 200 patients with the diagnosis of midshaft clavicle fracture based on clinical and radiological criteria. The clinical design was a classic Cohort study with measures 3 month and 6 month after Operative or Non-operative therapy. The responsiveness was evaluated based on the effect size (ES) and the standardrized response mean (SRM). Result: The Constant-Murley’s Shoulder instrument showed ES: 1.38 and SRM: 1.51 (large responsive). Discussion: The Constant Murley’s shoulder (CMS) instrument is sensitive in detecting clinical change for use in clinical outcome studies of midshaft clavicle fracture done at 3 month and 6 month after operative or non-operative therapy.
