EVALUASI PENGGUNAAN ANTI BIOTIKA PRE DAN POST OPERATIF PADA TINDAKAN PEMBEDAHAN DI BANGSAL BEDAH INSTALASI RAWAT INAP RUMAH SAKIT UMUM DAERAH ULIN BANJARMASIN PERIODE SEPTEMBER-NOPEMBER 2006

bakteri, reaksi hipersensitivitas, superinfeksi dan Ketidak tepatan pemberian antibiotika pada kasus pembedahan akan meningkatkan resistensi memperpanjang masa perawatan (Setiawan, 2003). Penelitian ini bertujuan untuk mengevaluasi penggunaan antibiotika pre operatif dan post operatif pada tindakan pembedahan di bangsal Instalasi Rawat Inap RRUD Ulin Banjarmasin. Treatment Guidelines From The Medical Letter Penelitian ini merupakan penelitian non eksperimental dengan rancangan deskriptif evaluatif melalui penelusuran data secara prospektif terhadap rekam medik pasien rawat inap RSUD Ulin Banjarmasin periode September – Nopember 2006. Permasalahan yang akan dievaluasi adalah kesesuaian pemilihan antibiotika baik pre dan post operatif dengan Standar Pelayanan Medik bagian Bedah dan , (2009) serta Antibiotic Prophylaxis in Surgery (SIGN, 2008), kesesuaian dosis, cara dan lama pemberian dengan Drug Information Handbook (2011-2012) serta efektivitas penggunaan antibiotika. Antibiotika pre dan post operatif yang banyak digunakan Cefotaxim 1 gr (32 dan 25 k Karakteristik 95 pasien (75 orang laki laki, 20 orang perempuan), usia terbanyak 16-60 tahun (49 orang), operasi kotor (59 orang), operasi elektif (67 orang), operasi kurang dari 1 jam (74 orang). asus). Kesesuaian dengan Standar Pelayanan Medik pada pre operatif 65 kasus (68,24%) dan pada post operatif 63 kasus (66,31%), sedangkan dosis, cara dan lama pemberian yang sesuai dengan Drug Information Handbook (2011-2012) pada pre operatif 54 kasus (56,84%) dan pada post operatif 46 kasus (48,42%). Kesesuaian dengan Treatment Guidelines From The Medical Letter, (2009) serta Antibiotic Prophylaxis in Surgery (SIGN, 2008), pada pre operatif 58 kasus (61,05%) dan post operatif 59 kasus (62,11%), sedangkan dosis, cara dan lama pemberian yang sesuai dengan Drug Information Handbook (2011-2012) pada pre operatif 48 kasus (50,53%) dan pada post operatif 42 kasus (44,21%). Efektivitas penggunaan antibiotika pre operatif lebih baik pada yang tidak sesuai Standar Pelayanan Medik maupun Treatment Guidelines From The Medical Letter, (2009) serta Antibiotic Prophylaxis in Surgery (SIGN, 2008) dengan kasus infeksi hanya 7 pasien (7,39%). Treatment Guidelines From The Medical Letter, (2009) Kesesuaian penggunaan lebih tinggi dengan Standar Pelayanan Medik dari pada dengan baik pada pre operatif maupun post operatif, kejadian infeksi masih tinggi (38 orang) dan meninggal 2 orang.

Conformity with as well as antibiotics Prophylaxis in Surgery (SIGN, 1999) in pre operative 58 cases (61.05%) and post-operative 59 cases (62.11%), while the dose, how and duration of which in accordance with the Drug Information Handbook (2011-2012) on the pre operative 48 cases (50.53%) and on post-operative 42 cases (44.21%). The effectiveness of pre-operative use of antibiotics is better in an inappropriate or Treatment Guidelines From The Medical Letter, (2009) as well as Antibiotics Prophylaxis in Surgery (SIGN, 2009) with only 7 cases of infection patients (7.39%). Treatment Guidelines From The Medical Letter, (2009) Compliance is higher with the use of Standards of Medical Care in the as well as Antibiotics Prophylaxis in Surgery (SIGN, 2009) both at the pre operative and post operative, incidence of infection is still high (38 persons) and died 2 people.
