LAMA KERJA ANALGESI PADA BLOK SUBARAHNOID BUPIVAKAIN 0,5 % HIPERBARIK DENGAN TAMBAHAN MORFIN 0,05 MG PADA PENANGANAN NYERI 24 JAM PASCA SECTIO CAESAREA

Penelitian observasional dengan desain cross sectional ini bertujuan melihat lama kerja analgesi obat tambahan morfin 0,05 mg pada blok subarahnoid (SAB) bupivakain 0,5% hiperbarik untuk mengatasi nyeri 24 jam pasca section caesarea (SC). Subyek adalah 86 pasien perempuan yang menjalani SC dengan anestesi regional SAB di IRD RS Dr. Sardjito, usia 20- 40 tahun, status fisik ASA I dan II, lama operasi tidak lebih dari 120 menit, berat badan 40- 70 kg, tinggi badan 150- 170 cm (BMI 17,5- 24,5 kg/ m 2 ). Pada penelitian ini subyek penelitian berjumlah 86 pasien, masing –masing pasien dilakukan SAB dengan obat anestesi lokal bupivakain 0,5% hiperbarik ditambah morfin 0,05 mg. Pungsi lumbal dilakukan pada posisi duduk, penyuntikan pada ruang intervertebra L3-4, dengan jarum spinal ukuran 25 G. Seluruh pasien diberikan cairan prabeban kristaloid 10 mL/kgBB pada 30 menit sebelum SAB. Hasil utama penelitian ini adalah lama kerja analgesi morfin 0,05 mg sebagai tambahan bupivakain 0,5% hiperbarik pada SC dengan SAB, hasil lama kerja analgesi pada kelompok < 24 jam berjumlah 30 pasien (34,9%) dan pada kelompok ≥ 24 jam berjumlah 56 pasien (65,1%) Kesimpulan : SAB bupivakain 0,5% hiperbarik tambahan morfin 0,05 mg pada SC mempunyai lama kerja analgesi< 24 jam sebanyak 30 pasien (34,9%) dan ≥ 24 jam sebanyak 56 pasien (65,1%). Pengukuran utama : skala VAS nyeri pasca operasi.

This observational study with cross sectional methode purposed on observing analgetic duration of 0,05 morphine adjuvants in Subarachnoid Block (SAB) with hyperbaric 0,5% bupivacaine in order to overcome 24 hours post Caesarean Section operations pain. The subjects were eighty six female patients who underwent Caesarean Section with Subarachnoid Block (SAB) regional anaesthesia in Dr. Sardjito Hospital emergency room, age ranged from 20 to 40 years old, American Society of Anaesthesiologists (ASA) I and II physical status, duration of surgery less than 120 minutes, with body weight started between 40 to 70 kg, and height between 150 to 170 cms (BMI 17,5 to 24,5 kg/m 2 ). In this study, the study subjects consisted of 86 patients; each patient underwent SAB with hyperbaric 0,5% bupivacaine local anaesthesia added with 0,05 mg morphine. Lumbar punction have been done in sitting position, injected on L3-4 inter vertebral space, with 25 G sized spinal needle. Ten millilitres crystalloid preload liquid for each kilogram of body weight have been shed 30 minutes before SAB to all of the patients. The main result of this study was stressed upon analgetic duration of 0,05 morphine as an adjuvant into hyperbaric 0,5% bupivacaine in Caesarean Section operations with Subarachnoid Block (SAB), there were 30 patients (34,9 %) in group with analgetic duration less than 24 hours; while in equal or more than 24 hours group there were 56 patients (65,1 %). Conclusion: Subarachnoid Block (SAB) with hyperbaric 0,5 % bupivacaine and 0,05 mg morphine as adjuvant on Caesarean Section had analgetic activity duration less than 24 hours resulted in 30 patients (34,9 %) while in equal or more than 24 hours group resulted in 56 patients (65,1 %). Main Variables: Post operative pain Visual Analogue Scale (VAS).
