This study aims to obtain empirical evidence from six dimensions of success in implementation of information system SAP R/3 in Sinarmas Forestry. The six dimensions of Information System (IS) Success are System Quality, Information Quality, Service Quality, User Satisfaction, Individual Impact, and Organizational Impact. According to models developed by DeLone and McLean, and Pitt et al., that there is a relationship between variables System Quality, Information Quality, Service Quality, User Satisfaction, Individual Impact, and Organizational Impact. This study also tries to find out the relationship between the six dimensions. Data used in this study are obtained by using a questionnaire from the SAP User who can represent various levels of the organization, office location, department, educational background and length of experience in using information system. This study found that implementation of SAP R/3 systems at Sinarmas Forestry has low level of success. This study also found that level of success in managerial user is higher than non managerial user and level of success in Head Quarter is the highest level, followed West Kalimantan Region, South Sumatera Region, East Kalimantan Region, Jambi Region and Riau Region. By department, level of success in Budget Monitoring is the highest level, followed by Finance, HRD, Internal Audit, Operation, Logistic, Business Control, and Accounting. The regression equation shows the relationship between dependent variable, User Satisfaction, and three independent variables: System Quality, Information Quality, and Service Quality, are jointly statistically significant. In partially regression test, all the regression coefficients are individually statistically significant. The relationship between dependent variable, Individual Impact, and four independent variables: System Quality, Information Quality, Service Quality and User Satisfaction are jointly statistically significant, and in partially regression test, all the regression coefficients are individually statistically significant. The relationship between dependent variable, Organizational Impact, and five independent variables: System Quality, Information Quality, Service Quality, User Satisfaction and Individual Impact are jointly statistically significant, and in partially regression test, all the regression coefficients are individually statistically significant
