Validitas foto posisi waters pada trauma facial

Salah satu aspek menentukan dalam praktek kegawatdaruratan adalah penatalaksanaan pasien dengan trauma facial. Perlukaan ini seringkali menyulitkan petugas gawat darurat untuk menilainya karena kekurangpengetahuan akan anamnesis dan teknik pemeriksaan maxillofacial. Radiografi facial sering dilakukan untuk menyingkirkan fraktur midfacial, seringkali 2 sampai 4 proyeksi Tujuan penelitian ini adalah untuk mengetahui validitas foto kepala posisi Waters pada fraktur trauma facial. Kami menilai 51 subyek penelitian trauma facial yang melakukan pemeriksaan foto kepala posisi Waters dan CT scan kepala. Penelitian ini dilaksanakan di Instalasi Radiologi RSUP Dr. Sardjito sejak bulan Mei sampai dengan Juli 2010. Didapat 30 subyek penelitian laki-laki dan 21 subyek penelitian perempuan dengan rentang usia antara 3 tahun sampai dengan 69 tahun (rerata 31.3 tahun). Foto kepala posisi Waters dinilai oleh seorang dokter spesialis radiologi dan CT scan kepala dinilai oleh dokter spesialis radiologi lain. Derajat kesesuaian (kappa) intra-observer dihitung untuk masing-masing dokter spesialis radiologi.Hasil penelitian menunjukkan sensitivitas 87%; spesifisitas 70%; rasio kemungkinan positif 2,9; rasio kemungkinan negatif 0,19; nilai prediksi positif (NPP) 0,82; nilai prediksi negatif (NPN) 0,78 and akurasi 80%. Foto kepala posisi Waters terbukti memiliki validitas yang baik dalam penapisan diagnosis trauma facial di Instalasi Gawat Darurat.

One of the most demanding aspects of emergency practice is the management of the patient who has suffered facial trauma. For emergency officers these injuries often difficult to evaluate due to insufficient undergraduate training in maxillofacial history taking and examination techniques. Facial view radiographs are often requested to exclude a midfacial fracture, commonly comprise two to four views.This study is to find out the validity of Waters view radiograph to evaluate fractures in facial trauma patients. We evaluated 51 patients, who received both Waters view radiograph and head CT scan due to trauma. This study was held in Radiology Department of Dr. Sardjito General Central Hospital from Mei to Juli 2010. There were 30 males and 21 females with the age ranging from 3 to 69 years (mean 31.3 years). The Waters view radiograph was evaluated by a radiologist and head CT scan was evaluated by other radiologist. Intra-observer kappa was calculated for each radiologist.The result showed sensitivity of 87%, specificity of 70%, positive likelyhood ratio of 2,9, negative likelyhood ratio of 0,19, positive predictive value (ppv) of 0,82,negative predictive value (npv) of 0,78, and accuracy of 80%. Waters view radiograph is indeed valuable as a screening examination in the diagnosis of facial fractures for trauma patients at Emergency Department.
