Analisis biaya perawatan gagal ginjal kronis rawat inap sebagai pertimbangan dalam penetapan pembiayaan kesehatan berdaasr Ina-DRG di RSUD Dr Moewardi

Penerapan sistem INA-DRG bagi pasien kelas III Jamkesmas di rumah sakit mempunyai tujuan agar pelayanan kesehatan menjadi efektif dan efisien baik dalam hal perencanaan pengobatan maupun biaya. Tujuan penelitian adalah menganalisis biaya perawatan pasien gagal ginjal kronis, mengetahui pengaruh faktor pasien (usia dan jenis kelamin), penyakit penyerta dan lama perawatan terhadap biaya perawatan GGK dan mengetahui perbedaan biaya rata-rata perawatan GGK sesuai tarif RSUD Dr Moewardi dengan tarif paket INA-DRG. Jenis penelitian adalah deskriptif menurut perspektif rumah sakit. Metode pengambilan data dilakukan secara retrospektif. Subyek penelitian adalah pasien rawat inap di RSUD Dr. Moewardi periode Mei 2009 - November 2009 yang menderita gagal ginjal kronis dan memenuhi kriteria inklusi. Subyek yang digunakan dalam analisis biaya pengobatan gagal ginjal kronis hanya dibatasi pada pasien kelas 3 peserta Jamkesmas dengan periode yang sama. Variabel meliputi karakter demografi (usia dan jenis kelamin), penyakit penyerta, lama perawatan, biaya perawatan (direct medical cost dan direct non-medical cost) dan serta outcome. Analisis data dilakukan secara cross tab dan linier regression. Hasil penelitian menunjukan bahwa tidak ada hubungan antara karakteristik pasien dan penyakit penyerta dengan biaya perawatan di RS, sedangkan lama waktu perawatan mempunyai hubungan dengan biaya perawatan. Rata-rata biaya perawatan gagal ginjal kronis pada tingkat keparahan 1,2 dan 3 lebih rendah dibandingkan dengan tarif paket INA-DRG sedangkan perbedaan rata-rata LOS riil dan rata-rata LOS INA-DRG hanya terjadi pada tingkat keparahan 3.

Application of INA-DRG system for Class III of Jamkesmas’s patients in the hospital has the objective to health services become more effective and efficient both in terms of treatment and cost planning. The research objective was to analyze the cost of care of patients with chronic renal failure, find out the relationship between patient factors (age and gender), concomitant disease and length of stay with the cost of treatment and know the difference between the average cost of treatment of CKD according Dr Moewardi Hospital tariff and tariff INA –DRG’s package. The study was descriptive according hospital's perspective. Data is taken by using retrospective method. Subjects were inpatients at Dr. Moewardi Hospital within period May 2009 - November 2009 who suffered from chronic renal failure and fulfill the inclusion criteria. The subject is limited on class III of Jamkesmas’s patient with same period. Variables include demographic characteristics (age and gender), concomitant diseases, length of stay, costs of treatment (direct medical costs and direct non-medical costs), and outcome. Data analysis done by using crosstab, linier regression. Results showed that there was no relationship between patient characteristics and concomitant diseases with the cost of care in hospital, while the length of stay had a relationship with the cost of treatment. Average treatment costs in chronic kidney failure severity 1, 2 and 3 lower than the tariff package INA-DRG, while the average difference in the real LOS, and average LOS INA-DRG severity levels only occurred in three.
