Pengaruh pemekaran wilayah terhadap tingkat efektifitas pelayanan :: Studi kasus pelayanan kesehatan di Kabupaten Penajam Paser Utara

Pemekaran wilayah ditujukan pada peningkatan pelayanan kepada masyarakat, seperti yang terjadi di Kabupaten Penajam Paser Utara. Disahkannya Undang-undang Nomor 7 Tahun 2002 tentang Pembentukan Kabupaten Penajam Paser Utara (PPU), mengakibatkan secara resmi terbentuk Kabupaten PPU yang merupakan pemekaran dari Kabupaten Pasir. Kabupaten PPU sebagai kabupaten baru, perlu segera dilengkapi dengan perangkat kelembagaan, prasarana dan sarana pemerintahan, serta perencanaan pembangunan, salah satunya adalah pelayanan kesehatan. Penelitian ini bertujuan untuk menganalisis perkembangan pembangunan bidang kesehatan di Kabupaten Penajam Paser Utara dan sejauh mana pemekaran wilayah mempengaruhi tingkat efektivitas pelayanan kesehatan.Penelitian dilakukan dengan menggunakan pendekatan deduktif kualitatif, metode analisis triangulasi dan deskriptif.Hasil penelitian menyimpulkan bahwa secara kuantitas; dengan pemekaran wilayah dari dua belas kecamatan menjadi empat kecamatan yang membentuk Kabupaten Penajam Paser Utara menyebabkan jumlah penduduk menjadi berkurang karena luas wilayah menjadi lebih kecil, namun kepadatan menjadi lebih tinggi, jumlah fasilitas kesehatan menjadi berkurang. Walaupun mengalami pengurangan tetapi sudah dirasa memenuhi rasio dengan jumlah penduduk. Secara kualitas; berkurangnya jumlah fasilitas kesehatan bukan berarti bahwa mengalami penurunan kualitas, justru dengan berkurangnya jumlah fasilitas dan kesesuaian dengan jumlah penduduk berarti fasilitas dapat termanfaatkan maksimal. Lebih lanjut, dengan pemekaran wilayah, maka jarak semakin dekat dan waktu tempuh semakin cepat, sehingga untuk menuju pusat-pusat pelayanan bisa lebih efektif dan efisien.Tingkat efektivitas pelayanan kesehatan tercapai jika fasilitas tersedia dan berkesinambungan, mudah dicapai, dan bermutu, prosentase ketercapaian pelaksanaan program/kegiatan bidang kesehatan dapat terealisasi sebesar 100%.Pemekaran wilayah berpengaruh pada tingkat efektivitas pelayanan kesehatan tetapi tidak pada semua aspek. Hal ini dibuktikan bahwa pelayanan tercapai bukan dalam segi kenampakan fisik melainkan dalam bentuk pelayanan yang diberikan tenaga medis melayani pasien, kemampuan mengatasi penyakit atau menyembuhkan pasien, serta kemampuan sosialisasi terhadap pentingnya kesehatan.

The main purpose from regional proliferation to add services for humanity like in Penajam Paser Utara Regency. The enactment of Law No. 7/2002 on the Formation of Penajam Paser Utara Regency, Penajam Paser Utara Regency was legally formed as a regional proliferation of Pasir Regency. As a new region,Penajam Paser Utara Regency needed to be equipped with institutions, facilities and infrastructures as well as development planning. The study focuses on how to analyze the progress of development in health sectors at Penajam Paser Utara Regency and to analyze the extent to which the regional proliferation influences the degree of effectiveness in health care service the research used deductive qualitative approach, triangulation method, and descriptive comparison before and after the regional proliferation. The study found that in terms of quantity, the splitting of four of the twelve subdistricts forming the areas of Penajam Paser Utara Regency led to the decrease in the number of population to cover by the health care services since the coverage area becomes smaller. However, there was an increase in the degree of population density increase and an decrease in the number of health facilities. Despite the decrease, the number of health facilities is considered sufficient to cover the population ratio. In terms of quality, the decrease in the number of health facilities does not necessarily mean the decrease in the quality of service. Instead, the number of health facilities is proportional to the number of population. It means that the facilities can be utilized in maximal way. In addition,the regional proliferation implies closer geographical proximity and decrease in the time required to get to the health service centers. Effectiveness in the health service is realized when the facilities are available and sustainable, accessible, and qualified. A percentage of 100% in the achievement of health programs/activities can be realized. Regional proliferation influences the degree of effectiveness in health service even though comparable degree of success is not evenly achieved at all aspects. Achievement in health care service is not in terms of physical appearance. Instead the achievement is in the way the health care professionals serve the patients, in the ability to treat the disease or recover the patients, and in the ability to socialize the importance of health care and to socialize the principles that prevention is better than cure.
