Evaluasi penerapan SPMKK terhadap program pelayanan puskesmas di 3 Provinsi Yogyakarta, Jawa Tengah dan Kalimantan Timur

Latar Belakang: Profesionalisme perawat dan bidan mendapat perhatian yang rendah dalam organisasi pelayanan puskesmas. Badan Kesehatan Dunia dan Kementerian Kesehatan Indonesia telah mengembangkan kebijakan perbaikan profesionalisme dan kinerja perawat dan bidan sejak 2001. Kebijakan ini dilaksanakan sejak tahun 2002 sampai sekarang. Seluruh daerah telah mendapat pelatihan dan pengembangan di bidang ini. Penelitian ini mempelajari praktik yang dihasilkan oleh kebijakan pelatihan SPMKK untuk kesehatan ibu dan anak. Tujuan Penelitian: Penelitian ini melakukan evaluasi terhadap dampak pelatihan terhadap kinerja perawat dan bidan. Selanjutnya, secara khusus ingin mengkaji peran dari manajemen puskesmas dan dinas kesehatan dalam mendukung program nasional ini di tingkat puskesmas. Metode Penelitian: Penelitian cross sectional dilengkapi dengan wawacara mendalam. Data yang dikelolah merupakan data skunder Evaluasi Penerapan SPMKK di sembilan puskesmas di tiga provinsi Yogyakarta, Jawah Tengah dan Kalimatan Timur terhadap Peningkatan Kualitas Kinerja Puskesmas yang dilakukan oleh Menteri Kesehatan tahun 2009. Hasil: Penelitian in menunjukkan bahwa kehadiran SPMKK di bagian pelayanan kesehatan ibu dan anak di puskesmas dirasakan cocok dengan upaya pengembangan profesionalisme keperawatan dan kebidanan. Meskipun demikian, terdapat variasi dalam tingkat praktik dari masing-masing komponen kinerja. Praktik monitoring serta diskusi refleksi kasus merupakan kinerja yang paling rendah di antara 5 komponan SPMKK. Penelitian ini juga menunjukkan bahwa masing-masing puskesmas dan dinas kesehatan daerah memiliki peran dalam menunjang kinerja keperawatan dan kebidanan. Puskesmas di daerah DIY dan Jawa Tengah memperlihatkan kinerja yang relatif lebih baik dari pada di daerah Kalimantan Timur. Kesimpulan dan saran: Hasil penelitian ini membuktikan bahwa praktik SPMKK berhubungan dengan peran dukungan dari puskesmas dan dinas kesehatan daerah. Penelitian ini menyarankan bahwa bidan dan perawat yang telah dilatih memperoleh kesempatan untuk mengembangkan diri atas dukungan kepala puskesmas. Jika mereka memperoleh fasilitasi untuk meneruskan hasil workshop SPMKK yang telah mereka lalui, maka pengembangan kinerja klinik itu akan terus berkembang. Dinas kesehatan provinsi dapat mendukung lebih lanjut dengan continuing education di tempat dan kesempatan mengambil pelajaran dari daerah lain. Dinas provinsi memiliki informasi yang menjadi dasar evaluasi program kinerja bidan dan perawat. Pemerintah daerah harus didukung agar bisa memberikan dukungan yang terus-menerus dalam menfasilitasi SPMKK ini.

Background. The professionalism of midwives and nurses get a low attention in health service program. World Health Organization and the Ministry of Health of Indonesia has developed a policy of improving the professionalism and clinical performance of nurses and midwives since 2001. Implementation of the policy was taken between 2002 until the present time. District health offices have implemented the training and development of the program. This research is to study the midwife and nurse practices as the result of CPDMS training. Because the focus of global policy now is the reduction of maternal and child mortality, this study focuses on MCH health services. This study aimed at evaluating the impact of training on the performance of nurses and midwives. It examines the role of health centers and health service management in support of this national program at the health center level. Method. This cross sectional study was also equipped with in-depth interview and uses a survey data of midwives and nurses at nine clinics in East Kalimantan, Central Java and Yogyakarta Special Region. Secondary data was obtained through the research ever undertaken by the Ministry of Health from February until March 2009. Result. This research indicates that the health workers training and workshop fit the perceived effort to develop the professionalism of nursing and midwifery. Nevertheless, there are variations in the level of practice of each performance component. Monitoring and case reflection practices are the lowest score among five component of CPDMS. This study also showed that each of these health centers and local health department have the role in supporting the performance of nursing and midwifery. Health centers in areas of Yogyakarta and Central Java showed relatively better performance than in the East Kalimantan region. Conclusions and recommendations. The results of this study prove that SPMKK practices associated with the role of support from health centers and local health department. This study suggests that midwives and nurses should have an opportunity to develop themselves. If they get to continue the facilitation of a workshop SPMKK they have been through, then the development of clinical performance that will continue to grow.The provincial health office can support further by the continuing education. Opportunity to take lessons from other regions. Provincial Department has information for performance evaluation programs and nurse midwives. Local governments should be advocated to provide ongoing support in facilitating this CPDMS.
