Rancangan database dalam pengembangan rekam medis elektronik rawat jalan di Rumah Sakit Panti Rapih Yogyakarta

Latar Belakang. Rumah sakit berkomitmen untuk memberikan pelayanan kesehatan yang bermutu, mengatasi masalah rekam medis dengan menerapkan rekam medis elektronik berbasis komputer pada poliklinik rawat jalan yang dapat mendukung keputusan klinisi dan manajemen rumah sakit. Permasalahan keterlambatan rekam medis, pelayanan gigi lama, resep lama, tempat penyimpanan rekam medis penuh diharapkan dapat diatasi dengan menjalankan rekam medis elektronik ini. Sebelumnya telah ada pilot project pada klinik syaraf dan akan dilanjutkan pada KSM Gigi. Tujuan. Penelitian ini bertujuan untuk Mengidentifikasi Skema Database dan Entity Relationship Diagram yang dibutuhkan Dokter dan Manajemen serta Mengidentifikasi manfaat Rekam Medis Elektronik sebagai pendukung keputusan bagi Klinisi dan manajemen Rumah sakit. Metode Penelitian. Rancangan penelitian ini menggunakan metode kualitatif dengan pendekatan action research yang bertujuan untuk mengeksplorasi kebutuhan untuk database dalam tahapan pengembangan Rekam Medis Elektronik Rawat Jalan pada KSM gigi. Pengambilan sampel dengan cara Non Probabilitas dengan teknik pengambilan sampling Purposive. Cara pengumpulan data dilakukan dengan melakukan Observasi dengan menggunakan pedoman observasi dan melakukan pengumpulan data. Hasil. Hasil yang diperoleh dari penelitian intervensi ini adalah berupa rancangan database yang menghasilkan 8 tabel data khusus klinik gigi. Implementasi Rekam medis elektronik ini dapat mengatasi masalah (1) Keterlambatan dokumen rekam medis; (2) Pelayanan gigi lama; (3) Pelayanan resep lama; (4) rekam medis dipakai secara bersamaan; (5) Kesiapan sumber daya di rumah sakit dalam merancang rekam medis elektronik. Kesimpulan. Perancangan database Rekam Medis Elektronik Rawat Jalan pada KSM Gigi dapat memenuhi harapan yang diinginkan dokter dan manajemen rumah sakit untuk mengatasi masalah di rawat jalan dan kecepatan output di klinik gigi; Rancangan sistem yang telah disusun dapat dimanfaatkan untuk mendukung keputusan dokter dan manajemen rumah sakit; Perancangan dan pengembangan rekam medis elektronik mudah digunakan dan sesuai kebutuhan organisasi dan pengguna.

Background. Panti Rapih General Hospital has a commitment to give a quality service, to overcome problems on medical record by applying the computer-based medical record at the outpatient clinic which supports the decision, made by clinicians and the management. There has been a pilot project at Neurology Clinic and it will be applied at Dental Clinic Objective:This research aims at identifying Database Scheme and the diagram of the entity relationship needed by the doctors and the management and identifying the benefit of the computer-based medical record to support the decision made by the clinicians and the management of the hospital. Research Method: This research employed a qualitative method with action research approach which aims at exploring the needs of database which is needed for the development of the computer-based medical record at the outpatient clinic of the Dental Medical Staff Group. The sampling used was non-probability method with purposive sampling. It data colection involved interviews and observations. Result: The result of the research is in the form of database which produces 8 table data for dental clinic. The implementation of the computer-based medical record shows that the system is able to solve problems due to (1) delayed document completion, (2) extended process of dental service, (3) extended process of the pharmacy services, (4) shared uses of medical record by different departments at the same time, (5) the readiness of the human resource in developing electronic-based medical record. Conclusion: The design of the computer-based medical record database at Dental outpatient clinic is able to fulfill the expectation of the doctors and the management to solve the problems on the outpatient clinic and on the immediacy of the information output at dental clinic; the system developed can be employed to support the decision made by the doctors and the management; the electronic-based medical record is easy to use and fits the needs of the organization and the users
