Kadar cystating C penderita hipertensi di Kecamatan Mlati Yogyakarta

Cystatin C merupakan penanda baru dan sangat sensitif sebagai penanda laju filtrasi glomerulus. Penurunan laju filtrasi glomerulus ringan dapat ditentukan dengan mudah menggunakan serum cystatin C daripada dengan kadar serum kreatinin. Tujuan penelitian ini adalah untuk mengetahui perbedaan kadar serum cystatin C pada penderita normotensi dan hipertensi di Kecamatan Mlati Kabupaten Sleman, Yogyakarta. Metode yang digunakan uji potong lintang dan acak stratifikasi dari 12.073 subyek yang berasal dari 36 dusun dari 39 dusun pada 3 desa di kecamatan Mlati, Kabupaten Sleman. Dari 12.073 didapatkan kelompok normotensi sebanyak 3.491 subyek, prehipertensi 6.210 subyek, dan hipertensi 2.372 subyek. Sampel yang diambil pada penelitian ini dibatasi usia antara 30-59 tahun (didapatkan hasil normotensi 2.082 subyek dan hipertensi 1.522 subyek). Kemudian dilakukan acak terstratifikasi untuk mendapatkan 100 subyek (50 subyek dari normotensi dan 50 subyek dari hipertensi). Rerata usia untuk kelompok normotensi adalah 42±7,9 tahun dan hipertensi 46±7,4 tahun (p=0,66). Terdapat perbedaan bermakna rerata kadar cystatin C kelompok normotensi 0,68±0,09 mg/dL dan kelompok hipertensi 1,03±0,09 mg/dL (p=0,0001). Tidak terdapat perbedaan kadar kreatinin (0,76±0,18 dibanding 0,84±0,19; p=0,367), ureum (26±8,02 dibanding 23,3±6,4; p=0,219), dan IMT (21±3,05 dibanding 22±4,3; p=0,081) pada kelompok normotensi dibandingkan kelompok hipertensi. Kadar serum cystatin C secara bermakna lebih tinggi pada kelompok hipertensi dibandingkan dengan normotensi.
The hypertension is a risk factor for increased rate of morbidity and mortality resulted from cerebrovasculare and particularly end stage renal disease. Cystatin C is a new marker and the most sensitive marker of glomerular filtration rate. A mild reduction in glomerular filtration rate is detected more easily by serum cystatin than serum creatinine level. The aim of this research is to know the difference of cystatin C level in normotension and hypertension patient in Mlati subdistric, Sleman district, Yogyakarta. The method used is cross-sectional study with stratification randomized sampling from 12,073 persons of 36 subvillages from 39 sub-villages in three villages in Mlati subdistrict, Sleman District. From 12,073 samples, it was found 3,491 normotension subject, 6,210 prehypertension subject, and 2,372 hypertension subject. The sample that were recruited in this study was limited by age range of 30-59 years old (2,082 subjects are normotension and 1,522 subject are hypertension). And than the recruited sample was stratification randomized for getting 100 subject (50 subjects are normotension anf 50 subjects are hypertension). The mean of age for normotension group is 42±7.9 years old, and 46±7.4 years old for hypertension group. The mean of cystatin C level is 0.68±0.09 mg/dL in normotension group and 1.03±0.09 mg/dL in hypertension group. There is a significant difference between normotension group and hypertension group (p=0.0001). The difference between body mass index (BMI) in normotension group and hypertension group is not significant (21±3.05 vs 22±4.3; p = 0.081). The difference between serum creatinine level and ureum level in normotension group and hypertension group is not significant (0.76±0.18 vs 0.84±0.19; p=0.367 and 26±8.02 vs 25±6.4; p=0.219). The level of serum cystatin C is significantly higher in hypertension patients than normotension patiens.
