Implementasi informed consent pada pelayanan persalinan oleh bidan desa di Kabupaten Kotawaringin Timur

Undang-Undang Nomor 36 Tahun 2009 tentang Kesehatan telah mengatur petugas kesehatan agar memberikan informed consent kepada pasien sebelum melakukan tindakan pelayanan kesehatan. Informed consent sangat penting dalam pelayanan persalinan karena pelayanan persalinan merupakan pelayanan kesehatan yang berisiko. Oleh karenanya perlu evaluasi pelaksanaan informed consent dalam pelayanan persalinan yang ditolong oleh bidan desa. Tujuan. Untuk mengetahui implementasi informed consent pada pelayanan persalinan oleh bidan desa di Kabupaten Kotawaringin Timur. Metode. Jenis penelitian ini adalah deskriptif dengan rancangan studi kasus dan menggunakan metode pengumpulan data secara kualitatif. Subyek penelitian ini adalah Dinas Kesehatan dan Ikatan Bidan Indonesia (IBI) Kabupaten Kotawaringin Timur serta Puskesmas dan bidan desa kategori perkotaan, daerah terpencil dan sangat terpencil. Data dikumpulkan dengan wawancara mendalam, diskusi kelompok terarah dan cek dokumen. Hasil. Informed consent belum dilaksanakan karena informed consent belum dicantumkan dalam Standar Operasional Prosedur (SOP) pelayanan persalinan bidan desa, sehingga bidan tidak melanggar standar yang telah ditetapkan. Di sisi lain, masyarakat tidak menuntut adanya informed consent karena masyarakat tidak memahami manfaat informed consent. Bidan hanya mengetahui manfaat dan fungsi Informed consent sebagai bukti hukum, namun belum terampil dalam penerapannya karena bidan lebih mementingkan pelayanan persalinan dari pada administrasi persalinan. Pembinaan dari Puskesmas dan Dinas Kesehatan maupun organisasi profesi belum dijalankan secara optimal. Kesimpulan. Bidan tidak melakukan informed consent akibat Dinas Kesehatan yang kurang responsif terhadap UU kesehatan sehingga bidan tidak termotivasi untuk mengerjakannya.
Background. Act number 36 in 2009 about Health has regulated the medical staff to give informed consent for the patients before giving health service action. Informed consent is important in delivery service because delivery service is a kind of health service with high risk. Therefore, it is necessary to evaluate the implementation of informed consent in giving delivery service helped by midwife in village. Aims. To know the implementation of informed consent in giving delivery service by midwife in village in Kotawaringin Timur Regency. Methods. The study uses descriptive method by case study design and by qualitative method in collecting data. The subject of research is Health Minister and Ikatan Bidan Indonesia (IBI), of Kotawaringin Timur Regency and Public Health Center, midwife of village of city cathegory,outlying district and under outlying district. The data is collected by effective interview, well-design group discussion, and checking the document related. Results. Informed consent has not been performed because informed consent do not belong to Procedure Operational Standard of delivery service of midwife in village, in result that the midwife of village do not break the act or law determined. In other side, the society do not demand the informed consent because they have less understanding about the benefit of informed consent itself. The midwife only recognize the benefit and function of informed consent as the law of evidence, however they are not able to apply it because they only focus on the delivery service given than the delivery administration. Training coming from Public Health Center, Health Minister and health profession organization is not optimaly done. Conclusion. The midwife has not applied informed consent because the Health Minister is less responsive towards the Act of Health therefore the midwife has less motivation to implement it.
