Analisis biaya terapi pasien rawat inap jamkesmas di RSUD Panembahan Senopati Bantul tahun 2008

Jamkesmas adalah program bantuan sosial untuk pelayanan kesehatan bagi masyarakat miskin dan tidak mampu. Tujuannya adalah agar terjadi subsidi silang dalam rangka mewujudkan pelayanan kesehatan yang menyeluruh bagi masyarakat miskin. Setiap peserta Jamkesmas berhak mendapatkan pelayanan kesehatan tanpa dikenakan iur biaya dengan alasan apapun. Jika ada selisih harga diluar jenis paket dan tarif pelayanan kesehatan maka pemerintah daerah berkewajiban untuk menanggung. Maka perlu dilakukan analisis biaya pasien rawat inap Jamkesmas tahun 2008. Tujuan penelitian ini adalah untuk mengetahui karakteristik pasien, hasil verifikasi Jamkesmas, selisih rata-rata biaya terapi dan selisih rata-rata biaya obat.Penelitian menggunakan rancangan deskriptif non eksperimental yang diambil dengan metode stratified proportional random sampling secara retrospektif. Data diambil dari bagian rekam medis, kasir, dan bagian keuangan dengan 5 diagnosis utama yang memiliki kriteria inklusi. Data dikelompokkan berdasarkan diagnosa dan dianalisis secara deskriptif dengan menggunakan pendekatan rata-rata biaya terapi.Hasil penelitian didapatkan data untuk appendicitis yang memenuhi kriteria inklusi sebanyak 37 kasus, GEA 33 kasus, hipertensi 31 kasus, ISK 32 kasus, dan ISPA sebanyak 38 kasus. Berdasarkan hasil verifikasi pasien Jamkesmas rawat inap tahun 2008, jumlah total klaim Rp 6.152.995.376 jumlah klaim layak bayar Rp 6.053.265.909 dan jumlah klaim yang tidak layak bayar Rp 119.729.467. Selisih biaya terapi rata-rata dari hasil verifikasi akhir Jamkesmas dengan biaya medik langsung dalam kuitansi pada penyakit appendicitis sebesar Rp 56.028 ± 299.686, penyakit GEA sebesar Rp -12.266 ± 204.275, penyakit hipertensi sebesar Rp -118.697 ± 329.572, penyakit ISK sebesar Rp 155.582 ± 696.428, dan penyakit ISPA sebesar Rp 86.417 ± 156.541. Selisih biaya obat ratarata dari hasil verifikasi akhir Jamkesmas dengan biaya obat dalam kuitansi appendicitis sebesar Rp -424.818 ± 444.459, penyakit GEA sebesar Rp -31.110 ± 96.157, penyakit hipertensi sebesar Rp -67.001 ± 114.541, penyakit ISK sebesar Rp 41.635 ± 353.945, dan penyakit ISPA sebesar Rp 32.955 ± 132.815.

Jamkesmas is social assistance health services program to poor society.This program is aimed to make cross subsidy health services for all poor society.Each Jamkesmas participant deserves unconditionally free health services. If there is a discrepancy in that service types and tariffs, then the regional government is obliged to pay the claim. Hence, it is necessary to analyze the cost of Jamkesmas inpatient in 2008. This research is aimed at identifying the characteristics of patients, verification of Jamkesmas, average discrepancy of therapy cost, and average discrepancy of medicine cost.This research uses non-experimental descriptive design, which is using proportional random sampling method that collected retrospectively. The data is taken from the patient’s medical record, chasier, and accounting division. The data is classified based on the diagnose types and analized with descriptif analysis on average discrepancy of therapy cost .The result shows that appendicitis patient that need inclution criteria is 37 cases, of acutegastroenteritis (GEA) is 33 cases, of hypertension is 31 cases, of urinary tract infections is 32 cases , and of acute respiratory infection is 38 cases.The total amount of claims of Jamkesmas inpatient in 2008 is Rp 6.152.995.376,of the payable claims is Rp 6.053.265.909, and of the non-payable claims is Rp 119.729.467. The average discrepancy cost of Jamkesmas verification and direct medical cost on receipt of appendicitis therapy is 56.028 ± 299.686, of acutegastroenteritis (GEA) is Rp -12.266 ± 204.275,, of hypertension is Rp - 118.697 ± 329.572, of urinary tract infections is Rp 155.582 ± 696.428, and of acute respiratory infection is Rp 86.417 ± 156.541. The average discrepancy of medicine cost of Jamkesmas verification and medicine cost on receipt of appendicitis therapy is Rp -424.818 ± 444.459, of acutegastroenteritis (GEA) is Rp -31.110 ± 96.157, of hypertension is Rp -67.001 ± 114.541, of urinary tract infections is Rp 41.635 ± 353.945, and of acute respiratory infection is Rp 32.955 ± 132.815.
