Gambaran kualitas hidup (hasil akhir) pasien fraktur intertrochanter pasca pemasangan angled blade plate

Terjadinya fraktur proksimal femur meningkat seiring dengan peningkatan usia pada populasi. Kejadian fraktur intertrochanter mencapai 90 % dari keseluruhan fraktur proksimal femur pada pasien usia tua. Terapi pilihan pada fraktur intertrochanter adalah operatif dengan menggunakan angled blade plate . Angled blade plate mempunyai keuntungan diantaranya yaitu minimal soft tissue stripping. Tujuan penelitian ini adalah untuk mengetahui gambaran demografi fraktur intertrochanter, mengetahui hasil akhir pasien fraktur intertrochanter setelah pemasangan angled blade plate dengan alat ukur Harris Hip Score, mengetahui apakah umur, jenis kelamin,serta adanya penyakit penyerta mempengaruhi hasil akhir. Subjek penelitian adalah pasien – pasien fraktur inter trochanter tipe tidak stabil (klasifikasi evans) yang dilakukan ORIF dengan angled blade plate sebagai pilihan implant di RS.Dr.Sardjito DIY pada periode 1 Januari 2006 s/d 31 Desember 2008. Didapatkan 54 pasien, namun pasien yang masuk dalam kriteria inklusi penelitian hanya 33 pasien. Demografi pasien adalah 21 pasien laki – laki (63,6%) dan12 pasien wanita (36,4 %). Berdasarkan umur, diatas 80 tahun adalah (42,4 %) diikuti umur 70 – 79 tahun (27,3 %).Umur rata – rata 75,8 tahun dan range berkisar 55 tahun sampai 97 tahun. Kualitas hidup pasca tindakan dinilai dengan menggunakan Harris Hip Scor. Didapat rata – rata score 82,112 (baik), range antara 54 sampai 98. Sebelas pasien dengan nilai sangat baik (33,3 %), baik 9 pasien (27,3 %), cukup 4 pasien (12,1 %) dan buruk 4 pasien (12,1 %). Kesimpulan yang didapatkan dari penelitian adalah resiko terjadinya fraktur intertrochanter pada lansia semakin meningkat dengan bertambahnya umur pasien, ratarata pasien pada penelitian mempunyai hasil akhir 82,112 (Baik) dengan range 54 – 98. Tidak ada korelasi antara hasil akhir pasca pemasangan angled blade plate terhadap umur dan jenis kelamin pasien.Ada korelasi antara hasil akhir yang buruk pasca pemasangan angled blade plate terhadap ada tidaknya penyakit penyerta pada pasien. Angled blade plate dapat digunakan sebagai salah satu pilihan implant untuk fiksasi fraktur pada fraktur intertrochanter dengan hasil akhir rata – rata adalah baik.

Incidence of Proximal third femoral fractures increase along with life expectancy. Intertrochanteric fractures reach 90% from overall fractures of the proximal third fracture in elderly. Choices of treatment in intertrochanteric fracture are operatively. The usage of angled blade plate which have advantages including minimal soft tissue stripping. The objective of this study is to observe demographic incidence of intertrochanteric in Yogyakarta, to understand the outcomes of the intertrochanteric fracture after angled blade plate insertion with Harris Hip Score, to know the correlation between age, sex and also comorbidity affects the outcomes. Subject of the study were all patients with unstable intertrochanteric fractures (Evans classifications) which underwent ORIF with angled blade plate on Sardjito General Hospital in Yogyakarta from January 1st, 2006 to December 31th, 2008. Quality of life assessment for post operative patient use Harris Hip Score. Fifty four patients were observed, only 33 patients inclusions criteria. Sex distribution of the patients were 21 male (63,6%) and 12 female (36,4%) with ratio 1,74 : 1 higher ratio in male. In age distribution more than 80 y.o (42,4% ), mean age was 75,88 y.o with range of age between 55 – 97 y.o. Outcomes quality of life post operatively with ORIF angled blade plate with mean score 82,1 (good), with range score 54 – 98 in patients with very good score ( 33,3% ) .Nine patient were good ( 27,3% )and 4 patients fair ( 12,1% ) Conclusion of this research is the risk of intertrocanteric fracture in elderly was increased, average outcomes for these patients is 82,112 (good) with range 54 – 98. No correlation between fair outcome in post surgery using angled blade plate between age and sex patients. There are correlation between the fair result of post operatively with angled blade plate and patient comorbidity. Angled blade plate can be use an implant choice for the intertrochanteric fracture with good average outcomes.
