Evaluasi pelaksanaan sistem pencatatan dan pelaporan terpadu Puskesmas di Kabupaten Karimun

Oleh karena itu perlu dilakukan penelitian untuk mengkaji faktor-faktor yang mempengaruhi pelaksanaan Sistem Pencatatan dan Pelaporan Terpadu Puskesmas (SP2TP) di Puskesmas agar dapat memberikan gambaran nyata dan diharapkan dapat dimanfaatkan untuk pengambilan keputusan dalam upaya perbaikan dan peningkatan program. Tujuan: Secara umum penelitian ini bertujuan untuk mengevaluasi pelaksanaaan SP2TP di Kabupaten Karimun. Adapun tujuan khusus penelitian ini adalah mengeksplorasi faktor input pada pelaksanaan SP2TP, mengeksplorasi faktor proses pada pelaksanaan SP2TP dan mengeksplorasi faktor output pelaksanaan SP2TP di Kabupaten Karimun. Metode Penelitian: Penelitian ini merupakan penelitian diskriptif evaluatif dengan rancangan studi kasus (case study) dengan menggunakan metode kualitatif. Subjek penelitian ini berjumlah 11 orang yang terdiri atas kepala puskesmas dan koordinator SP2TP dan Dinas Kesehatan Kabupaten Karimun. Sampel penelitian adalah puskesmas Durai, puskesmas Kundur Barat, dan puskesmas Tanjung Balai Karimun. Hasil Penelitian: Input pelaksanaan SP2TP di Kabupaten Karimun adalah untuk kebijakan belum ada SK dan juklak/juknis pelaksanaan SP2TP,dana tidak pernah ada, SDM masih kurang, khususnya puskesmas Durai, banyak petugas yang rangkap jabatan dan teknologi yang ada belum membawa perubahaan pelaksanaan SP2TP. Untuk proses SP2TP adalah pengumpulan data terlambat karena data tidak diarsip rapi, pengiriman laporan terdapat kendala transportasi. Adapun analisis, penyajian, kualitas pengecekan data dan umpan balik jarang dilakukan bahkan untuk puskesmas terpencil tidak pernah dilakukan. Output SP2TP ketepatan waktu laporan sering terlambat, keakuratan data laporan tidak akurat khususnya puskesmas terpencil, laporan tidak lengkap sehingga data disusulkan lewat SMS. dan pemanfaatan untuk perencanan dan evaluasi SP2TP juga belum dilakukan secara optimal. Kesimpulan: Pelaksanaan sistem pencatatan dan pelaporan puskesmas di Dinas Kesehatan Kabupaten Karimun sudah terlaksana meskipun secara kualitas baik input, proses maupun outputnya belum optimal. Hal tersebut karena belum adanya dukungan SDM baik secara kualitas dan kuantitas maupun kondisi geografis bagi sebagian puskesmas daerah terpencil yang sangat sulit terlebih lagi belum adanya dana operasional untuk pelaksanaan SP2TP di Kabupaten Karimun.
Background : Implementation of the Integrated Recording and Reporting System of the Public Health Center (SP2TP) is just limited to data, which are results of an interaction between community and health facility. The SP2TP can also help in planning the health programs of the public health center. However, it was unable to implement as expected, even the presence of the SP2TP was seen as a relatively burdening one for staff in the public health center. Therefore, evaluation must be done to study the implementation of the SP2TP and to find problems technically and nontechnically faced in the public health center, so that it can provide a real picture of the SP2TP in the public health center and can be expected to utilize in a decision making. Objectives: This study aimed to evaluating the SP2TP implementation at Karimun regency. The specific objectives are explore the implementation of SP2TP input factors, factors in the implementation process, the implementation of SP2TP output factors in Karimun regency. Method: This is a evaluative descriptive study with a case study design using a qualitative method to explore relevant information as much as possible and the process of implementing the SP2TP of the Public Health Center. Subjects in this study were 11 people consisting of Head of Public Health Center, executive coordinator SP2TP and Karimun District Health Office. Techniques used in determining the sample is purposive sampling. The research sample are Durai, West Kundur, and Tanjung Balai Karimun public health center. Research Results: Inputs: there is no decree policy and operational or technical guidelines for SP2TP implementation, the funds did not exist, human resources are still lacking, especially Durai public health, many officers who double position, technological changes have not yet brought SP2TP implementation. SP2TP process are data collection is too late for archived data neat, delivery of reports have transportation barriers; analysis, presentation, quality checks and feedback are rarely done. Outputs are timeliness of reporting is often too late, the accuracy of reporting is inaccurate data, especially remote health centers, the report is incomplete, so the data continuation is sent by SMS. and utilization for planning and evaluation SP2TP have not done optimally. Conclusion: The SP2TP of the Public Health Center has actually been implemented in Karimun Regency, but the quality was relatively low, because the implementation was sufficiently not supported by the human resource both in quality and quantity and geographic conditions for many the Public Health Center are very difficult even more so the lack of operational funds for the implementation of SP2TP in Karimun regency.
