Explorative study of aircraft accident and serious incident investigation results in Indonesia

Aviation safety measures have been a subject of improvement in Indonesia, in line with the vision and mission of the Department of Transportation. Several strategies and punishments have also been provided to service providers who disobey the regulation. However, the number of occurrences during the past six years has showed an increasing trend. By using Empirical Bayesian method, airports are classified according to similarity of their characteristics, function, and operation. Predicted and expected numbers of accident are calculated. Deviations from the predicted number of accidents are identified. Types of accidents and serious incidents are analysed based on their phases of flight. The number of accidents per year is divided by the number of flights per year to get the aviation safety level. This thesis is aimed to identify the most hazardous airport in Indonesia, to analyse occurrences by phase of flight and their types, to map level of safety, and to provide safety recommendation for safety improvement. Five most hazardous airports are identified. Furthermore, the data showed that safety problems at those airports mostly are kites, airport surroundings, weather, birds, and dogs. The number of occurrences occurring in landing phase are found to be 204 occurrences (42%), followed by en route of 116 occurrences (24%,) and take-off phase of 68 occurrences (14%). Types of occurrences on landing phase are runway excursion (28%), abnormal runway contact (17%), and system/ component failure or malfunction – non-power plant (17%). On en route phase there are system/ component failure or malfunction – power plant (26%), unknown (23%), and system/ component failure or malfunction – non-power plant (18%). On take-off phase there are system/ component failure or malfunction – non-power plant (24%), unknown (20%), system/ component failure or malfunction – power plant (19%), and runway excursion (14%). Finally, according to accident rate calculation, it is identified that aviation safety level in Indonesia, from 2004 to 2008, ranges from 2,16E-5 to 4,12E-5.
