Analisis penerapan formularium terhadap persediaan obat untuk pasien umum RSUD Tugurejo Semarang

Pelayanan farmasi rumah sakit merupakan salah satu kegiatan di rumah sakit yang menunjang pelayanan kesehatan yang bermutu. Sebagai rumah sakit milik pemerintah RSUD Tugurejo menghadapi masalah dalam pelayanan obat yaitu adanya dokter yang menulis obat diluar obat yang tersedia di rumah sakit, sehingga tidak menutup kemungkinan resep yang keluar dari rumah sakit, yang pada akhirnya pelayanan kesehatan yang bermutu tidak dapat terpenuhi. Selain itu masalah yang sering terjadi di RSUD Tugurejo yaitu adanya penggantian obat dan kekosongan obat. Adanya formularium rumah sakit, diharapkan dapat meningkatkan efisiensi dan efektivitas dalam pengelolaan persediaan obat mulai dari perencanaan, pengadaan dan distribusi serta dapat meningkatkan pendapatan rumah sakit lewat instalasi farmasi rumah sakit dengan meningkatnya jumlah resep yang terlayani, dan semakin sedikit resep yang keluar dari rumah sakit. Penelitian bertujuan untuk mengetahui penerapan formularium pada pasien umum terhadap persediaan obat di RSUD Tugurejo Semarang dan mengevaluasi tingkat kesesuaian resep dokter terhadap formularium. Penelitian merupakan penelitian deskriptif bersifat retrospektif dan concurent. Data penelitian diperoleh dari wawancara dengan Kepala Instalasi Farmasi, Ketua Panitia Farmasi dan Terapi, serta kuesioner kepada dokter sebanyak 55, laporan stok akhir tahun 2006 dan 2007, laporan penerimaan obat tahun 2007, resep tahun 2007, daftar obat dalam formularium tahun 2007. Analisis data disajikan dalam bentuk persentase yang kemudian dikaji dengan hasil wawancara kepala IFRS dan ketua PFT. Hasil penelitian menunjukkan bahwa penerapan Formularium Pasien Umum di Rumah Sakit Umum Tugurejo Semarang cukup efektif. Rasio perputaran obat yang dicapai yaitu 6,94 kali. Tingkat kesesuaian resep dokter terhadap formularium di RSUD Tugurejo Semarang sebesar 98,70%. Sosialisasi yang terus dilakukan, komunikasi yang lancar, serta menjalin hubungan profesional yang harmonis antara dokter dan farmasis, juga ikut mempengaruhi tingginya tingkat kepatuhan dokter terhadap formularium.

The hospital pharmacy service is one of the hospital’s activities to support the high quality health care. As a government’s hospital, RSUD Tugurejo face a problem in medicine care since there is doctor who give the medicine prescription out of medicine that are available in Tugurejo hospital, so that it’s possible that the prescription can be taken out from Tugurejo, and this caused the high quality health care cannot be fulfilled. Besides, the problem that often happened in RSUD Tugurejo is the replacement and or the emptiness of the medicine. Hospital’s formularium, is expected can improve the efficiency and effectiveness in medicine’s supplying management begins from planning, supplying and distribution and also can improve the hospital’s income through the hospital pharmacy’s installation in the improvement of prescription served, and the prescription which are taken out from hospital are decrease. This research is aim to know the applying of formularium to medical patient toward the medicine’s supply in RSUD Tugurejo Semarang and to evaluate how far the obedience of the doctors to formularium. The type of research is descriptive retrospective and concurent. Data are taken from interview with the Pharmacy Installation head, the chief of Committee of Pharmacy and Therapy, and also 55 questioners to doctors, year-end of 2006 and 2007 report stock, report of medicine acceptance in 2007, prescription in 2007, lists of the medicine in formularium in 2007. The Analysis of the data is presented in percentage form which is later studied with the result of the interview with the head of IFRS and the chief PFT. Result of research indicated that the application of medical patient Formularium in RSUD Tugurejo Semarang is effective enough. The medicine rotation ratio reached is 6.94 times. The obedience level of the doctor toward formularium at RSUD Tugurejo Semarang is 98.70%. A routine socialization, smooth of communications and also the harmonious relation among doctor and pharmacy, is also influenced the obedience level of the doctor toward formularium.
