Pengaruh standarisasi penggunaan bahan habis pakai untuk bedah sesar pasien jaminan kesehatan masyarakat di instalasi bedah sentral RSUD Kadinah Tegal

Instalasi Bedah Sentral (IBS) sebagai sarana penunjang kegiatan staf medis fungsional, adalah penyedia semua sarana dan tenaga yang diperlukan oleh staf medis fungsional agar tindakan medis dan operasi yang dilakukan berjalan dengan lancar, tertib, sehingga pasien mendapat pelayanan yang sempurna, memenuhi kepuasan pasien sesuai dengan standar profesi yang ditetapkan. Tujuan penelitian ini adalah melakukan standarisasi bahan habis pakai (BHP) bedah sesar pasien Jaminan Kesehatan masyarakat (JAMKESMAS) untuk meningkatkan efektivitas dan efisiensi, rasionalisasi serta menilai pengaruh standarisasi BHP bedah sesar pasien JAMKESMAS terhadap tingkat pendapatan yang diperoleh unit pelayanan farmasi (Yanfar) Instalasi Bedah Sentral (IBS) RSU Kardinah Tegal. Penelitian ini dilakukan dengan metode penelitian kuasi eksperimental, rancangannya adalah one group pre and posttest design. Data dikumpulkan secara retrospektif dan prospektif di unit Yanfar IBS RSU Kardinah Tegal. Sampel penelitian adalah jenis dan jumlah BHP 50 pasien JAMKESMAS sebelum intervensi dan 50 pasien sesudah intervensi tahun 2008. Dilakukan analisa pengaruh efektivitas dan efisiensi pemakaian BHP sebelum dan sesudah intervensi, kemudian masing-masing data mengenai jumlah dan jenis BHP bedah sesar yang berupa tingkat pendapatan dihitung dan dibandingkan antara sebelum intervensi dan sesudah intervensi dan dianalisa tingkat kebermaknaannya dengan uji t-test. Hasil penelitian, pada tahap pelaksanaan intervensi diketahui adanya kebocoran dan pemborosan penggunaan BHP untuk bedah sesar enam orang pasien JAMKESMAS sebesar Rp 500.237,00. Tahap setelah intervensi yang berupa penerapan standarisasi penggunaan BHP untuk bedah sesar dengan sistem paket, dapat menekan pemborosan dan kebocoran administrasi, adanya peningkatan pemakaian BHP, peningkatan efektivitas dan efisiensi serta peningkatan kerasionalan penggunaan BHP untuk bedah sesar pasien JAMKESMAS di IBS RSU Kardinah Tegal. Standarisasi berpengaruh menurunkan secara bermakna tingkat pendapatan Unit Yanfar IBS RSU Kardinah Tegal.

Central Surgery Installation as a unit that supports functional medical staff activities provides all facilities and staff needed by functional medical staff in order that medical intervention and operation run smoothly and correctly according to professional standard specified for patient satisfaction.Objective: The study aimed to standardize non durable materials of caesarean section of patients supported by community health insurance (JAMKESMAS) to improve effectiveness and efficiency, rationalization and assess the effect of non durable material standardization of caesarean section patients supported by JAMKESMAS to revenue obtained by pharmaceutical service unit of Central Surgery Installation of Kardinah Hospital Tegal.Method: The study used a quasi experiment method and one group pre and post test design. Data were obtained retrospectively and prospectively at pharmaceutical service unit of Central Surgery Installation of Kardinah Hospital Tegal. Samples of the study were types and amount of non durable materials used by 50 JAMKESMAS patients before intervention and 50 JAMKESMAS patients after intervention in 2008. Analysis of the effect of effectiveness and efficiency of the use of non durable materials before and after intervention was made. Then data of the amount and types of non durable materials for caesarean section in terms of revenue were calculated and compared between before and after intervention and their significance was analyzed using ttest. Result: At the stage of intervention implementation there were some leakage and wastefulness in the use of non durable materials for 6 patients of JAMKESMAS as much as Rp 500,237.00. At the stage after intervention in the form of standardization in the use of non durable materials for caesarean section using package system, leakage and wastefulness in administration could be minimized, there was increase of use of non durable materials, increase of effectiveness, and efficiency and increase in the rationality of the use of non durable materials for caesarean section in JAMKESMAS patients at Central Surgery Installation of Kardinah Hospital Tegal. Standardization significantly affected the decrease of revenue of pharmaceutical service unit of Central Surgery Installation of Kardinah Hospital Tegal.
