Penyimpanan dan pelayanan obat di tempat dokter praktek swasta

Telah dilakukan penelitian terhadap pengelolaan obat di tempat dokter praktek swasta. Penelitian bertujuan untuk melihat sejauhmana pengelolaan obat di tempat dokter praktek swasta meliputi pengadaan, penyimpanan dan penggunaan obat. Penelitian dilakukan di Kota Yogyakarta. Sampel penelitian dipilih dengan teknik non random sampling, yaitu dokter yang melakukan praktek swasta, perseorangan/kelompok, bukan di sarana pelayanan kesehatan, melakukan dispensing dan tinggal atau praktik kedokteran di Kota Yogyakarta. Data diambil menggunakan pedoman wawancara/interview dengan mengunjungi tempat praktek dokter. Data dianalisa secara on going process karena analisa data kualitatif merupakan upaya yang berlanjut, berulang dan terus-menerus. Hasil penelitian diperoleh bahwa terdapat dokter yang tidak memiliki STR atau tidak memiliki SIP serta tidak memiliki keduanya (STR dan SIP) lebih 10%. Dari sisi ketersediaan obat injeksi di tempat dokter praktek terdapat 45,24% dokter gigi, 43,14%dokter umum dan 25,74% dokter spesialis menyediakan obat injeksi. Sedangkan obat non injeksi terdapat 10,32% dokter gigi, 44,12% dokter umum dan 37,62% dokter spesialis menyediakan obat non injeksi. Pengadaan obat diperoleh dari apotek, toko obat, medical representatif, dan PBF. Secara administrasi, pengadaan obat dilakukan dengan permintaan lisan dan dicatat penerima. Dokter tidak mengarsip faktur pembelian. Faktur pembelian dari PBF dan Medical representatif berupa nota polos. Faktur pembelian yang menggunakan kop apotek, sebagian besar tidak ditandatangani Apoteker Pengelola Apotek. Penyimpanan obat menjadi satu dengan arsip/buku-buku, kondisi kurang menjamin mutu dan stabilitas sediaan obat. Secara administrasi, penyimpanan obat tidak dilengkapi kartu stok untuk memonitor keluar masuknya barang/obat. Dari sisi pemakaian, obat digunakan untuk pasien dan dicatat pada rekam medis, walaupun terdapat 3,34% dokter yang tidak membuat rekam medis. Secara persfektif hukum dan etika kedokteran, tidak ada alasan bagi dokter dan/atau dokter gigi untuk melakukan penyimpanan dan pelayanan obat (dispensing) kecuali obat dalam bentuk injeksi/suntikan.

This research in managing drugs have been made at the doctors’ clinics. This research is for seeing how far the managing drugs at the site of the doctor’s clinic which includes buying, storing and using drugs at the site. This research is case study of storing and selling drugs at the doctor’s clinic which takes place in Yogyakarta. The sample of this research using the method of non random sampling to doctors who run a private clinic, personally and not at health institutions in Yogyakarta. The method of collecting data is interview by visiting the clinics. Then the data were analyzed through on going process for analyzing qualitative data is progressive and continuous. The results of the research were: there were some doctors who had no STR or SIP and both (STR and SIP), specially for dentist, specialist and general doctor with the figure of over 10 %. The facts of the stock of injected drugs at the clinics that could be found were as follows: 45,24% at the dentist’s, 43,14% the general doctors’ and 25,74% the specialist’s. For the non injected drugs: 10,32% the dentist’s, 44,12% the general doctor’s 37,62% the specialist’s. All kinds of the drugs has licenses from governmental health institution. There were 4 sources of the drugs namely from Pharmacies, drug store, medical representatives, and pharmaceutical whole sellers. The purchase were made by oral orders and being written by the receivers. The doctors made some notes for the unpaid purchases, but not for the paid ones. The purchase notes from the PBF or Medical Representatives had no head letters. The ones with letterheads were not being signed by pharmacy. The drugs were stored side by side to dirty books. Some of the cupboards were too old to be free from mice as indicated by animal excrement and also the broken packages of the drugs. The purpose of storing drugs stocks to stabilizing was not followed. There was no good administration for storing the drugs. The drugs were given/used by the doctor’s patients who visited the clinics and some medical records were made, but 3, 34% of the doctors did not. Being studied from the medical, social and law perspectives there were no reasons for the doctors storing and dispensing, except for the injection ones.
