Perbandingan ekspresi matrix metallo proteinase-1 (MMP-1) pada pterigium primer progresif dengan dan tanpa injeksi triamcinolone acetonide subkonjungtiva

Latar belakang : Patogenesis dari pterigium dan penyebab kekambuhan setelah tindakan bedah masih menjadi kontroversi. Salah satu dari teori patogenesis adalah proses radang yang menyebabkan terjadinya pterigium dan mempengaruhi kekambuhan. Matrix Metallo Proteinase-1 adalah salah satu enzim endopeptidase yang bila ekspresinya berlebihan dapat menghambat proses perbaikan dari kerusakan sel. Triamcinolone acetonide adalah anti radang yang diharapkan dapat membatasi ekspresi MMP-1 dalam keadaan yang sesuai untuk perbaikan luka. Tujuan : Membandingkan ekspresi Matrix Metallo Proteinase-1 (MMP-1) dengan injeksi triamcinolone acetonide subkonjungtiva pada pterigium primer progresif. Bahan dan cara : Pemeriksaan dilakukan terhadap 64 mata dari 32 mata pada kelompok dengan injeksi dan 32 mata pada kelompok tanpa injeksi. Ekspresi dari kedua kelompok diperiksa menggunakan imunohistokimia. Penilaian berdasarkan atas jumlah sel positif per 100 sel di dalam lima lapang pandang yang berbeda. Hasil : Ekspresi berlebihan pada kelompok tanpa injeksi lebih kuat dan tinggi dibandingkan pada kelompok dengan injeksi. Perbedaannya secara statistik bermakna (P< 0.05), ekspresi MMP-1 pada kelompok tanpa injeksi adalah 74.32 (± 15.57) dan pada kelompok dengan injeksi adalah 28.82 (± 14.17). Kesimpulan : Ekspresi dari MMP-1 pada kelompok tanpa injeksi triamcinolone acetonide subkonjungtiva lebih tinggi dibandingkan kelompok dengan injeksi pada pterigium primer progresif dan secara statistik berbeda bermakna.

Background : Pathogenesis of the pterygium and the cause of recurrence rate after surgical treatment still a controversy. One of the pathogenesis theory is an inflammation can cause the pterygium and influence the recurrences. Overexpression Matrix MetalloProteinase-1 is one part of endopeptidase enzyme which involve on the wound healing and remodelling can inhibit the recovery of cell damage. Triamcinolone acetonide is anti inflammation has been proposed to limit the expression of MMP-1 in proper situation as its function to heal the wound. Objective : To compare the expression of Matrix Metallo Proteinase-1 (MMP-1) with triamcinolone acetonide injection subconjunctiva in progressive primary ptergyum. Methods : A total of 64 eyes from 32 eyes with injection group and 32 eyes without injection group were examined. All specimens were collected. Expression of MMP-1 in two groups was detected using imunohistochemistry. Measurement by amount of positive cell per 100 cell in five different areas. Results : The positive expression was stronger and higher in without injection group than in with injection group. The difference was significant (P< 0.05), whereas the expression of MMP-1 in without injection group was 74.32 (± 15.57) and with injection group was 28.82 (± 14.17). Conclusion : The expression of MMP-1 in post injected triamcinolone acetonide group was lower than in non-injected group on the progressive primary pterygium and the difference was statistically significant.
