Analisis penetapan jasa pelayanan medis dalam kerangka penetapan pola tarif RS di RSU Dr. Soedono Madiun

Latar belakang. Pola penetapan jasa pelayanan medis di rumah sakit pemerintah belum ada pedoman. Hal ini menimbulkan banyak persepsi, sehingga pola penetapan jasa pelayanan medis yang berdampak pada besaran tarip pelayanan akan berbeda-beda pula. Aspek regulasi menetapkan bahwa tarip rumah sakit pemerintah khususnya pemerintah daerah, harus ditetapkan dalam bentuk Peraturan Daerah (Perda), dimana prosesnya membutuhkan waktu lama, sehingga pada saat penetapan belum tentu masih sesuai keadaan. Hal ini sangat mempengaruhi kebijakan manajemen rumah sakit dalam penetapan tarip pelayanan. Tujuan. Menganalisis komponen atau faktor yang mempengaruhi dan bagaimana cara penetapan jasa pelayanan medis dalam kerangka penetapan pola tarip. Metode Penelitian. Penelitian ini menggunakan metode kualitatif. Data diperoleh dengan wawancara mendalam. Hasil Penelitian. Kebijakan penetapan jasa pelayanan medis dalam hubungan penetapan tarip pelayanan adalah perlu adanya batasan sehingga besaran jasa pelayanan medis bukan tak terbatas. Implementasi pada tarip ditetapkan 15 sampai dengan 44 % dari total tarip. Dasar penetapan yang diterapkan saat ini hanya : kelas perawatan, kedaruratan, dan jenis tindakan. Harapan ke depan harus mempertimbangkan faktor keahlian dan kedaruratan sebagai faktor utama, sedangkan faktor penyulit, jenis tindakan, tindakan yang dilimpahkan, lama bekerja, tanggung jawab, dan lama waktu tindakan sebagai faktor penunjang. Penetapan jasa pelayanan medis dengan cara pembobotan masing-masing faktor tersebut. Kesimpulan. Persepsi dokter spesialis dalam pola penetapan jasa pelayanan medis ada beberapa pendapat, baik faktor yang mempengaruhi maupun cara menetapkannya. Faktor keahlian medis, kedaruratan medis, jenis tindakan medis, dan penyulit tindakan medis merupakan faktor-faktor yang harus diperhitungkan pada penetapan jasa pelayanan medis. Sedangkan faktor tanggung jawab atas pelimpahan tugas, lamanya melakukan tindakan medis, pengalaman atau lama bekerja dan faktor pelimpahan tugas merupakan faktorfaktor tambahan dalam penetapan jasa pelayanan medis. Penetapan besaran jasa pelayanan medis diharapkan mempertimbangkan faktor-faktor diatas secara proporsional, disamping itu harus memperhatikan faktor pesaing dan kemampuan daya beli masyarakat. Background. There is no certain guidance to establish medical service rates patterns in state hospitals. This will emerge a lot of perceptions so that the medical service rates patterns will influence the difference of each rate. Regulation aspects establish the rates of state hospital especially the local government, and its process would take a long time for debates before its establishment and when it was in process. It would influence the policy of hospital management to establish medical services rates Objectives. To analyze the influenced component or factors and how to establish the medical service rates in the frames of rates patterns establishment. Methods. This study is using qualitative methods. The data was collected through depth interviewing. Result. The policy of medical service rates establishment in the relationship with service rates establishment needs the limitation so that the amount of medical service rates don’t mean unlimited. The implementation on the rates established should be 15 to 44 % from the total amount. The basic establishment carried out today only based on the service, emergencies, kinds of actions classifications, but in the future must consider the skill factors and the emergencies as the main factors, and the handicaps factors, kinds of actions, actions delegated, duration of work, responsibilities and duration of actions as supporting factors.The establishment of medical service rates in the way of each payload factors. Conclusion. There are some ideas and factors influence the perceptions of doctor specialist in establishment of the medical service rates patterns, and even the way to establish it. Medical skills, medical emergency, medical treatments, and the handicap of medical treatments factors should be considered in establishing medical service rates patterns. While the responsibility for job orders, the duration of medical treatments, the experiences and the duration of work, and job orders factors are added to establish medical service rates patterns.The establishment of medical services rates should consider all the factors above proportionally.
