Analisis biaya terapi pasien jaminan pemeliharaan kesehatan masyarakat miskin (JPK-MM) di RS PKU Muhammadiyah Yogyakarta

Pelayanan Askeskin di RS PKU Muhammadiyah Yogyakarta dalam pelaksanaannya terdapat kendala tentang penggunaan obat. Kendala tentang penggunaan obat ini disebabkan karena adanya perubahan-perubahan kebijakan mengenai standar obat yang ditetapkan oleh PT. Askes. Penelitian ini dilakukan untuk mengetahui selisih biaya terapi bagi pelayanan kesehatan pasien Askeskin rawat inap periode Januari-Desember 2007 yang harus ditanggung oleh pihak RS PKU Muhammadiyah Yogyakarta. Penelitian dilakukan dengan mengikuti rancangan cross sectional. Data dianalisis secara deskriptif dengan pendekatan rerata biaya terapi pasien Askeskin periode Januari-Desember 2007 dengan 5 diagnosis penyakit terbesar, yaitu bedah fraktur, bedah sectio caesaria, bedah appendicitis, gagal ginjal kronik dan stroke tanpa memperhitungkan tingkat keparahan dari kasus yang diambil sebagai sampel. Hasil penelitian menunjukkan bahwa: 1)urutan terbesar selisih biaya terapi bagi pelayanan kesehatan pasien Askeskin rawat inap yang harus ditanggung oleh pihak RS PKU Muhammadiyah Yogyakarta, yaitu pada kasus stroke sebesar Rp 2.407.732,00, appendicitis sebesar Rp 2.273.882,00, sectio caesaria sebesar Rp 1.764.630,00, gagal ginjal kronik sebesar Rp 1.692.568,00, dan fraktur sebesar Rp 656.535,00; 2)urutan terbesar selisih biaya penggunaan bahan dan alat kesehatan habis pakai di depo rawat inap per hari, yaitu pada kasus fraktur sebesar Rp 15.795,00, gagal ginjal kronik sebesar Rp 12.977,00, appendicitis sebesar Rp 6.978,00, sectio caesaria sebesar Rp -1.069,00, dan stroke sebesar Rp -26.210,00; 3)urutan terbesar rerata biaya penggunaan obat per pasien bagi pelayanan kesehatan pasien Askeskin rawat inap yang harus ditanggung oleh pihak RS PKU Muhammadiyah Yogyakarta, yaitu pada kasus stroke sebesar Rp 859.223,00, appendicitis sebesar Rp 328.785,00, fraktur sebesar Rp 169.643,00, sectio caesaria sebesar Rp 143.552,00, dan gagal ginjal kronik sebesar Rp 19.979,00.

Health insurance for poor people, Askeskin, services in “PKU Muhammadiyah” hospital Yogyakarta have in practice some difficulties in medicine usage. These difficulties are brought about by changes in policy regarding standards of medicine set by PT. Askes. The research aims at finding the difference in “PKU Muhammadiyah” hospital Yogyakarta liability of therapy expenses for Askeskin holder inpatients for January-December 2007 period. The research is conducted in accordance with the cross sectional design. Data are analyzed descriptively, taking particular approach to average cost for Askeskin holder patients for the January-December 2007 period with a diagnose to five largest diseases, that is to say, fracture surgery, sectio caesaria surgery, appendicitis surgery, chronic renal failure and stroke, taking severity into no account in the cases taken as sample. Result of the study indicates that: 1)the difference of therapy cost for Askeskin holder inpatients that is liable to be paid by “PKU Muhammadiyah” hospital Yogyakarta are, in high-to-low cost order: stroke cases of Rp 2,407,732.00, appendicitis of Rp 2,273,882.00, sectio caesaria of Rp 1,764,630.00, chronic renal failure of Rp 1,692,568.00 and fractures of Rp 656,535.00; 2)the difference in the cost of material usage and disposable health kit in inpatient unit per day is, in high-to-low cost order: in cases of fracture of Rp 15,795.00, chronic renal failure of Rp 12,977.00, appendicitis of Rp 6,978.00, sectio caesaria of Rp -1,069.00 and stroke of Rp -26,210.00; 3)the average cost for medicine usage for health services for Askeskin holder inpatients that is liable to be paid by “PKU Muhammadiyah” hospital Yogyakarta per day are, in high-tolow cost order: stroke cases of Rp 859,223.00, appendicitis of Rp 328,785.00, fractures of Rp 169,643.00, sectio caesaria of Rp 143,552.00 and chronic renal failure of Rp 19,979.00.
