Kadar calprotectin pada sel neutrofil dan cairan sulkus gingiva penderita penyakit periodontal yang disertai karsinoma nasofaring akibat infeksi Epstein-Barr virus

Epstein-Barr Virus (EBV) adalah anggota dari herpes virus yang dikaitkan dengan etiologi karsinoma nasofaring (KNF) dan mempengaruhi patogenitas bakteri penyebab penyakit periodontal. Virus tersebut dapat menginfeksi neutrofil dan menurunkan respon imun pasien sehingga mempengaruhi keparahan penyakit periodontal. Calprotectin sebagian besar berada dalam sitoplasma sel neutrofil dan levelnya akan meningkat pada beberapa penyakit inflamasi, termasuk inflamasi jaringan periodontal yang ditandai dengan peningkatan kadar calprotectin pada cairan sulkus gingiva (CSG). Tujuan penelitian ini adalah untuk mengkaji pola perubahan kadar calprotectin yang ada pada sel neutrofil darah tepi dan pada CSG pasien KNF berdasarkan tingkat keparahan penyakit periodontal, serta mengkaji hubungan antara keparahan penyakit periodontal dengan kadar calprotectin pada sel neutrofil darah tepi dan CSG pasien KNF. Sebelas pasien KNF akibat infeksi EBV digunakan sebagai subjek dalam penelitian ini. Keparahan penyakit periodontal diukur pada setiap pasien menggunakan indeks PDI Ramfjord. Dari masing-masing pasien diambil sel neutrofil darah tepi dan CSG. Kadar calprotectin pada sel neutrofil darah tepi dan CSG diukur menggunakan metode ELISA. Hasil penelitian menunjukkan terjadi penurunan kadar calprotectin pada sel neutrofil darah tepi dan peningkatan kadar calprotectin pada CSG pasien KNF berdasarkan tingkat keparahan penyakit periodontal. Hasil korelasi Pearson menunjukkan adanya hubungan yang signifikan antara keparahan penyakit periodontal dengan penurunan kadar calprotectin pada sel neutrofil darah tepi pasien KNF (p<0,05). Di lain pihak, peningkatan status keparahan penyakit periodontal tidak berhubungan secara signifikan dengan peningkatan kadar calprotectin pada CSG pasien KNF (p>0,05). Kesimpulan dari penelitian ini adalah bahwa penurunan kadar calprotectin pada sel neutrofil darah tepi pasien KNF akibat infeksi EBV diikuti peningkatan keparahan penyakit periodontal dan peningkatan kadar calprotectin dalam CSG pasien KNF sebagai akibat dari peningkatan keparahan penyakit periodontal.

Epstein Barr Virus (EBV) is a member of herpes virus family which associated with nasopharingeal carcinoma (NPC) and mediated periodontopathogenicity. This viral can infect neutrophil and decrease the immune response that affects severity of periodontal disease. Calprotectin is largely confined in the cytosol of neutrophil and its level increases in some inflamatory disease, including periodontal disease that is marked by increasing calprotectin level in crevicular gingival fluid (GCF). The aim of this study is to evaluate the pattern of calprotectin level alteration in neutrophil of the peripheral blood and in GCF of NPC patients according to the severity periodontal disease and to evaluate the correlation of periodontal disease severity with calprotectin level in neutrophil of the peripheral blood and GCF of NPC patient. Eleven NPC patients related EBV were used as the subject of this study. The severity of periodontal disease was determined from each patients used PDI Ramfjord. Neutrophil of peripheral blood and GCF were obtained from each patient. Calprotectin level of neutrophil peripheral blood and of GCF were measured by using ELISA method. The results of this study showed that calprotectin level in neutrophil of the peripheral blood decreased and that in GCF increased along with the severity of periodontal disease in NPC patient. Pearson Correlation test demonstrated that there were a significant correlation between the severity of periodontal disease and decreasing calprotectin level in neutrophil of the peripheral blood of NPC patient (p<0,05). On the other hand, the increasing severity of periodontal disease had an unsignificant correlation with increasing calprotectin level in GCF of NPC patients related EBV (p>0,05). In conclusion, the decreasing level of calprotectin in neutrophil cells’s NPC patients are followed by increasing severity of periodontal disease, on the other hand the increasing level of calprotectin of NPC patients in GCF as the result of periodontal disease severity.
