Restorasi estetik kompleks pada empat gigi insisivus maksila pasca trauma

Laporan kasus ini bertujuan untuk menginformasikan penggunaan mahkota jaket porselin fusi metal pada keempat gigi insisivus maksila pasca trauma, yang dapat dilakukan dengan perawatan secara menyeluruh dan terpadu untuk dapat mengembalikan fungsi gigi. Gigi 22 pulpa nekrosis dilakukan perawatan saluran akar satu kunjungan dengan metode preparasi step back, gigi 12 nonvital pasca perawatan saluran akar dengan obturasi underfilling dilakukan perawatan ulang saluran akar satu kunjungan dengan metode preparasi step back. Gigi 21 vital edge to edge dilakukan pembuatan mahkota jaket porselin fusi metal serta gigi 11 pasak tuang goyang tanpa mahkota jaket dilakukan restorasi ulang mahkota jaket porselin fusi metal. Pada keempat gigi tersebut dilakukan pembuatan mahkota jaket porselin fusi metal dengan retensi inti pasak tuang kecuali pada gigi 21 vital edge to edge. Pada restorasi mahkota jaket porselin fusi metal warna, bentuk dan posisi gigi adalah faktor – faktor penting untuk dapat mengembalikan fungsi gigi, termasuk di dalamnya fungsi estetik. Hasil perawatan restorasi baik, saat dilakukan kontrol satu minggu setelah insersi mahkota jaket porselin fusi metal tidak terdapat keluhan rasa sakit, gigingiva sekitar gigi normal. Gigi dapat berfungsi dengan baik serta dapat mengembalikan rasa percaya diri pada pasien.

The aim of this report is to inform the use of porcelain jacket crown fused to metal of four maxillary insisivus teeth pasca trauma, by comprehensive and integrated treatment to achieve optimum function of these teeth. On tooth 22 which was in necrotic pulp condition, one visit root canal treatment with step back method preparation were done, tooth 12 which was non vital had been root canal treated with underfilling obturation, handled with one visit root canal retreatment applying step back preparation method. Tooth 21 was in vital edge to edge condition, was given porcelain jacket crown fused to metal, and on tooth 11 there was mobility on its custom dowel core without jacket crown, anticipated with re-restoration jacket crown porcelain fused to metal. There were application of jacket crown porcelain fused to metal on these four teeth with retention custom dowel core except on tooth 21 which is still vital edge to edge. Colours, shapes and positions of all these four teeth became main factors in this restoration process to gain good teeth function, included esthetical function. The restoration treatment showed a good result on control visit one week later, there was no pain symptom after insertion of jacket crown porcelain fused to metal, also gingival in normal condition. Teeth show good function and also raised self confidence of patient as well.
