Pada gigi immature dinding saluran akar lebih tipis dibandingkan saluran akar gigi yang matang, sehingga risiko terjadinya fraktur akar cukup besar. Laporan kasus ini bertujuan mengemukakan kasus pasien yang datang ke klinik RSGM Prof. Soedomo dengan gigi yang patah di tengah- tengah perawatan apeksifikasi. Gigi 21 didiagnosis gigi non vital dengan fraktur akar pada daerah sepertiga servikal disertai lesi periapikal. Pada kasus tersebut dilanjutkan perawatan apeksifikasinya dengan obturasi saluran akar menggunakan guta perca customized, kemudian dilakukan ekstrusi akar menggunakan braket ortodontik sebagai anchorage dan pengait kawat stainless steel yang dimasukkan ke dalam saluran akar yang diikatkan ke bar insisal menggunakan elastik ortodontik. Tahap selanjutnya adalah restorasi mahkota jaket porselin fusi metal dengan pasak fiber clear disertai pembukaan flap. Makalah ini membahas mengenai pertimbangan rencana perawatan yang dijalankan, termasuk perawatan ekstrusi akar, pembukaan flap, pemilihan jenis pasak untuk kasus dengan saluran akar yang lebar dan dinding saluran akar yang tipis dan restorasi akhir untuk menge mbalikan fungsi gigi. Hasil perawatan menunjukkan bahwa restorasi mahkota jaket porselin fusi metal dengan pasak fiber clear pada gigi insisivus sentral maksila immature pasca apeksifikasi dan ekstrusi dapat mengembalikan fungsi estetik, fonetik, mastikasi dan pemeliharaan kesehatan jaringan pendukung.

The risk of immature tooth to root fracture is high. This case report is about a patient who came to RSGM Prof. Soedomo with fractured anterior tooth that was being apexificated. Tooth 21 were diagnosed as non vital teeth with servical root fracture and periapical lession. The treatment for this case was root canal filling followed by extrusion of the tooth. A restoration using porcelain fused to metal crown with fiber post were used after extrusion. The aim of this case report was to determine whether fiber clear post and porcelain fused to metal crown could restore the estethic and mastication function of this patient. This case report discuss about treatment plan consideration including extrusion of the tooth, flap opening, choice of post and restoration of immature tooth. The result showed that restoration using porcelain fused to metal crown with fiber clear post in maxillary central incisor after apexification and extrusion were able to restore the estethic, phonetic and mastication functions of this patient.
