Proses Perencanaan dan Penganggaran Program Pencegahan dan Pemberantasan Malaria Terpadu pada Dinas Kesehatan Propinsi Nusa Tenggara Timur

2008 | Tesis | S2 Ilmu Kesehatan MasyarakatLatar Belakang: Propinsi Nusa Tenggara Timur merupakan daerah hiperendemik Malaria dengan tingkat kesakitan dan kejadian luar biasa yang tinggi, menunjukan bahwa Malaria masih merupakan, masalah kesehatan masyarakat yang utama. Untuk meningkatkan kinerja program P2 Malaria pada Dinas Kesehatan Propinsi Nusa Tenggara Timur, maka hal pertama dan utama yang perlu diperhatikan adalah aspek perencanaan dan penganggarannya secara terpadu. Di sisi lain terdapat hambatan struktur organisasi dengan tugas pokok dan fungsi yang tidak konsisten dijalankan sesuai ketentuan Perda yang ada, memerlukan pendekatan fungsi kepemimpinan yang handal dengan komitmen yang kuat serta dukungan komunikasi dan koordinasi antar pengelola program baik dalam Dinas Kesehatan maupun lintas sektor terkait lainnya. Tujuan Penelitian: mengetahui dinamika interaksi (proses) perencanaan dan penganggaran program P2 Malaria terpadu pada Dinas Kesehatan Propinsi Nusa Tenggara Timur selama ini. Metode Penelitian: jenis penelitian ini adalah deskriptif dengan metode kualitatif melalui pendekatan (rancangan) studi kasus. Dalam hal ini kasus yang diangkat adalah dinamika interaksi (proses) perencanaan dan penganggaran program P2 Malaria pada Dinas Kesehatan Propinsi Nusa Tenggara Timur. Hasil Penelitian: diperoleh gambaran struktur organisasi dinas yang inkonsisten antara tugas pokok dan fungsi dalam ketentuan Perda dengan implementasinya di tingkat program. Sementara itu fungsi komunikasi dan koordinasi antara pengelola program malaria secara pribadi cukup baik. Sedangkan dukungan dan ketegasan pimpinan mengatasi hambatan struktural sudah dilaksanakan dengan membentuk tim perencanan dan penganggaran kesehatan terpadu (P2KT). Hanya perlu didukung oleh surat keputusan yang formal. Perlu advokasi pada lintas sektor terkait untuk memperoleh dukungan dalam perencanaan dan penganggaran. Kesimpulan: terdapat struktur organisasi dinas yang rancu dalam penenganan program P2 Malaria. Perlu tetap dibina komunikasi dan koordinasi antar pengelola program baik dalam Dinas Kesehatan sendiri maupun lintas sektor. Di sisi lain dukungan pimpinan yang sudah ada dengan tim P2KT perlu dipertegas dengan surat keputusan.
Background: East Nusatenggara Province is hyperendemic area of Malaria with high level of illness and extraordinary event. It indicates that Malaria still constitutes the main problem of public health. To improve the performance of Prevention and Eradication Program for Malaria in Health Service of East Nusatenggara, first and foremost, the government needs to pay more attention to the aspect of integrated planning and budgeting. On the other hand, there is structurally-organizational impediment with main task and function that is inconsistently performed in accordance with the provisions in the existing local regulation. This is very much in need of reliable leadership-function approach. Also, this approach certainly requires strong commitment and is to be supported with communication and coordination between good program-manager both in Health Service and across other sectors involved. Objective: This research was directed to find out dynamic interaction (process) on integrated planning and budgeting programs for Prevention and Eradication of Malaria in Health Service, East Nusatenggara Province. Method: This is a descriptive research using qualitative method with casestudy design. Dynamic interaction (process) on integrated planning and budgeting program for Prevention and Eradication of Malaria in Health Service, East Nusatenggara Province, was to be studied. Result: The result suggests that organizational structure of Health Service is inconsistent with main task and function as found in the local regulation with its implementation in program level. While, the function of communication and coordination between the manager of malaria program, both in Health Service and cross sectors involved is performed well enough. On the other hand, the support and the decisiveness of the leadership to overcome structural impediment have been carried out with the formation of IHPB (Integrated Health Planning and Budgeting) team. The most importantly, the team must be enforced with formal Letter of Confirmation and the program has to be advocated across sectors involved in order to get support for the planning and budgeting. Conclusion: There is extraneous organizational structure in the handling of Prevention and Elimination of Malaria. Communication and coordination between program managers both in Health Service as well as across sectors involved should be improved. On the other hand, the existing support from leadership along with integrated IHPB must be confirmed with Letter of Confirmation.
