Efek kombinasi triamsinolon asetonida dan metotreksat terhadap aktivitas fibroblas keloid pada ekivalen kulit jangat

2008 | Tesis | S2 PPDS I - Ilmu Kedokteran KlinikPenelitian ini bertujuan untuk menguji efek kombinasi triamsinolon asetonida (TA) dan metotreksat (MTX) pada aktifitas fibroblas keloid melalui daya hambat kontraksi Fibroblast Populated Collagen Lattice (FPCL) yang belum pernah dilaporkan sebelumnya. Hipotesis yang diajukan adalah kombinasi TA dan MTX akan menghambat aktifitas fibroblas keloid lebih besar dibandingkan pemberian TA dan MTX secara tunggal. Penelitian ini akan menggunakan rancangan penelitian eksperimental sederhana paralel multi grup, yang memakai kultur fibroblas keloid passage 3 yang diambil dari 42 pasien. Fibroblas dibiakkan dalam kolagen tipe 1 dari buntut tikus (FPCL) yang dikenal sebagai ekivalen kulit jangat, sering dipakai untuk menilai aktifitas fibroblas. Fibroblas keloid dibagi menjadi 4 grup dan diterapi dengan TA 5; 10; 20 μM, MTX 1,75; 3,5;7 μM, kombinasi TA dan MTX dan kontrol negatif. Kontraksi FPCL sebagai indikasi aktifitas fibroblas dinilai dengan scion image software. Rerata luas FPCL diuji dengan memakai analisis varian satu jalan (ANOVA).
Triamcinolone acetonid (TA) intra lesion has been a standard treatments for keloids for many years due to their effect in inhibiting collagen synthesis and fibroblast proliferation. However, until now the clinical result were unsatisfied. Keloid flattening is slow and sometimes adverse reaction is occurred. Methotrexate (MTX) is a chemotherapeutic agent having an anti-proliferating effect which act as an anti folic acid. Because of its effect, MTX is potential to use in combination with TA for the treatment keloid. Fibroblast Populated Collagen Lattice (FPCL) is a dermal equivalent which is usually used for fibroblast activities measurement. To investigate the effect of TA and MTX combination on keloid fibroblast activities by inhibits FPCL contraction, which never been investigated before. A simple parallel multi groups experimental study will be conducted on 3rd passage keloid fibroblast culture, isolated from 42 patients, cultivated in collagen lattice (FPCL). Keloid fibroblast are divided into 4 groups, then treated with 5; 10; 20 μM TA, 1,75; 3,5; 7 μM MTX, combination of TA and MTX and a control negative. FPCL contraction indicating activities of fibroblasts is measured using scion image software. Mean of FPCL contraction is analyzed using one way ANOVA.
