Latar Belakang: Rumah Sakit dr. H. Moch. Ansari Saleh adalah unit Pelaksana Teknis Dinas Kesehatan Provinsi Kalimantan Selatan. Selama ini RS menerima subsidi dari pemerintah baik melalui dana APBD dan APBN untuk keperluan belanja rutin RS dengan adanya kebijakan askeskin pemerintah berusaha mengurangi subsidi RS untuk membantu kelompok masyarakat miskin. Untuk itu pihak manajemen RS berusaha untuk dapat meningkatkan pendapatan RS dengan cara melakukan perubahan tarif pelayanan yang didasari dengan perhitungan unit cost dan melihat kemampuan membayar masyarakat (Ability to Pay) dan kemauan membayar masyarakat ( Willingness to Pay). Tujuan Penelitian: Menghitung unit cost biaya akomodasi pada rawat inap RS. Dr. H. Moch. Ansari Saleh Banjarmasin dan Mendapatkan gambaran kemampuan dan kemauan membayar masyarakat terhadap tarif akomodasi rawat inap, serta besaran subsidi yang diberikan oleh pemerintah pada RS. Metode Penelitian: Penelitian ini adalah penelitian deskriptif dengan menggunakan rancangan penelitian studi kasus, data dalam bentuk kuantitatif. Variabel dalam penelitian ini adalah Variabel bebas (independent) terdiri dari : Unit Cost akomodasi rawat inap, ATP , WTP dan Variabel terikat (dependent) adalah : Usulan tarif Hasil dan Pembahasan : Perhitungan Unit Cost akomodasi rawat inap di hitung dengan metode real cost dengan hasi :l rawat inap umum, kelas VIP Rp. 177.271,-, Kelas I Rp. 84.972,- Kelas II Rp.76.949,- dan Kelas III Rp. 46.542,- rawat inap jiwa kelas VIP Rp.222.858,- Kelas I Rp. 77.178,- Kelas II Rp. 50.741,- dan Kelas III Rp. 34.078,-. Kemampuan Membayar (ATP) masyarakat Kota Banjarmasin berdasarkan data Susenas yang di olah dengan tiga pendekatan berkisar antara Rp. 87.535.2 s/d 476.142,- sedangkan rata-rata kemauan membayar (WTP) pasien diatas tarif sekarang. Ada tiga skenario mengenai usulan tarif akomodasi rawat inap umum dan jiwa dengan Komposisi subsidi yang berbeda yaitu Tarif yang berlaku sekarang dengan besar subsidi Rp. 1.254.552.226,- Tarif sama dengan besaran WTP besar subsidi Rp. 1.124.281.879,- dan tarif berdasarkan Kepmenkes besar subsidi Rp. 208.453.902,- Kesimpulan : Subsidi pemerintah masih diperlukan untuk menetapkan pola tarif RS mengingat WTP masyarakat yang rendah.

Background: Dr. H.M. Ansari Saleh Hospital is a technical Implementation Unit of Kalimantan Selatan provincial Health Office. So the hospital gets subsidy from the government either through local and national revenue and expenditure budget. With the esthablishment of health insurance for poor community, the government reduces hospital subsidy to help poor community. Therefore the hospital management tries to increase hospital revenue by changing tariff of services based on calculation of unit cost, ability to pay and willingness to pay of the community. Objective: To calculate unit cost of inpatient accommodation of dr. H. M. Ansari Saleh Hospital and get an overview on ability and willingness to pay of the community and the amount of subsidy provided by the government for the hospital. Method: The study was observational with cross sectional survey using quantitative data. The independent variables of the study were inpatient accommodation unit cost, ability to pay, willingness to pay whereas the dependent variable was tariff proposal. Result and Discussion: Unit cost of general inpatient accommodation for VIP was Rp 177,271, for class I was Rp 84,972, for class II was Rp 76,949 and for class III was Rp 46,542. Unit cost of mental inpatient accommodation for VIP was Rp 222,858, for class I was Rp 77,178, for class II was Rp 50,741 and for class III was Rp 34,078. Ability to pay of Banjarmasin municipal community based on data of national economic census analyzed using three approaches ranged between Rp 87,535 to Rp 476,142 whereas average willingness to pay of the community was above the present tariff but under the unit cost. There were 3 scevarious on general and mental inpatient accomodation tariff proposal with different subsidy composition, i.e. the present tariff with the amount of subsidy as much as Rp 1,254,552,226; tariff based on WTP with subsidy Rp 1,124,281,879 and tariff based on the decree of the Ministry of Health with subsidy Rp 208,453,902. Conclusion: subsidy from the government was still needed to determine tariff pattern of the hospital considering that WTP of the community was still relatively low.
