Diabetes Melitus Tipe 2 sebagai faktor risiko demensia vaskuler pada kasus stroke

Vascular dementia (VaD) defined as a decrease in cognitive function due to ischemic brain lesion, hypoperfusion or hemorrhage caused by cerebrovascular or cardiovascular disease as the second leading cause of dementia in elderly after Alzheimer’s disease in USA and European countries, but the most frequent form found in Asian countries. Some evidence stated that vascular disease had an important role on the development of dementia. Diabetes Mellitus is a condition where there is an increase in blood glucose level persistently above normal limit due to insulin deficiency or any other factor intervene with insulin action. Cerebral function depends on continuous glucose supply as the main energy source, that the change in blood glucose level will interfere its function To determine whether type 2 Diabetes Mellitus is a risk factor of vascular dementia This study used case control method, all patients with stroke underwent anamnesis, physical and laboratory examination, ECG, Head CT Scan, SS-IQCODE assessment and MMSE. We used NINDS-AIREN criteria to diagnose Vascular Dementia. There were eighty eight stroke patients with a mean age of 61,14±10.55 in case group and 60.20 ± 9.99 in control group. There were more subject from the age of ≥ 65 years. VaD were not related with vascular factors (dyslipidemia, hypertension, and smoking status), stroke pathology (ischemic/hemorrhage), or location. Diabetes Mellitus (OR 4.58, 95% CI 1.54-13.6, p= 0.006) and Heart disease (OR 6.01, 95% CI 1.52-23.82, p= 0.011) were related with Vascular dementia. Stratification of Diabetes Mellitus without heart disease as a risk factor for VaD was with OR 3.89, 95% CI 1.25-12.07, p= 0.015. Diabetes Mellitus subjects who were treated with Antidiabetic oral drugs were not significantly related to VaD This study determined that type 2 Diabetes Mellitus is a potential risk factor for Vascular Dementia in stroke patients
