Apeksifikasi menggunakan Mineral Trioxide Aggregate dan Bleaching intrakoronal disertai restorasi resin komposit pada gigi anterior maksila berjejal

Tujuan laporan kasus klinis ini adalah untuk mengetahui apakah apeksifikasi menggunakan Mineral Trioxide Aggregate (MTA) dan bleaching intrakoronal yang dilakukan pada gigi insisivus sentralis maksila fraktur 1/3 insisal, pulpa nekrosis, apeks terbuka disertai diskolorasi dapat mengembalikan bentuk dan warna gigi insisivus sentralis maksila. Pasien laki-laki 17 tahun datang ke RSGM FKG Trisakti untuk memperbaiki dan merapihkan gigi depannya yang berjejal dan patah ketika berusia 12 tahun.Diagnosis gigi insisivus sentralis maksila fraktur 1/3 insisal, pulpa nekrosis, apeks terbuka, diskolorasi dan berjejal. Prosedur perawatan, diawali dengan perawatan saluran akar menggunakan preparasi konventional, apeksifikasi menggunakan MTA, pengisian saluran akar dengan teknik customized guta perca dan bleaching intrakoronal menggunakan hidrogen peroksida 30 %. Restorasi resin komposit aktivasi sinar dengan inti pasak resin komposit aktivasi kimiawi pada gigi insisivus sentralis maksila. Selanjutnya pasien dikonsulkan ke bagian ortodonsi untuk merawat gigi berjejal. Hasil evaluasi klinis dan radiografis setelah 9 bulan menunjukan perbaikan kondisi periapikal, warna restorasi resin komposit tidak berubah, bentuk dan kontur gigi baik, kerapatan tepi baik, pasien merasa puas.

The aim of this case report was to evaluate the result of apexification using Mineral Trioxide Aggregate (MTA) and intracoronal bleaching on maxillary central incisor teeth with necrotic pulp, open apex and discolored,and crown fracture of 1/3 incisal. A 17 year old male referred to Trisakti Oral and Dental Hospital to seek his crowded anterior teeth. The treatment procedure was initiated with root canal treatment using conventional technic, apexification sung MTA, root canal obturation using customized gutta percha and intracoronal bleaching with 30% hydrogen peroxide, followed by placement of root canal post of chemical activated resin composite. The patient was then referred to Orthodontic Dept for his tooth crowding. The result of clinical and radiographic examination after 9 months showed and improving periapical condition, no discoloration of resin composite restoration, better tooth countour and good marginal fit and the patient feel satisfied.
