Penerapan regulasi pelayanan anestesi dan privileging perawat anestesi di RSUD Wirosaban Kota Yogyakarta

Latar belakang: DaIam keterkaitan good clinical governance pemerintah mempunyai peran penting dalam regulasi dengan tujuan menjamin keselamatan pasien. Undang – undang 29 tahun 2004 Praktek kedokteran dan Permenkes No. 512/MENKES/PER/IV/2007, dalam upaya surat izin praktek dokter spesialis harus mendapatkan lisensi, rekomendasi oleh organisasi profesi seperti IDSAI. Privileging mencerminkan implementasi dari lisensi baik sarana kesehatan maupun personalnya. Regulasi sangat penting untuk pelaksanaannya sehingga tanggung jawab dan tanggung gugat dapat terinci. Tujuan: Mengetahui tingkat implementasi regulasi pelayanan anestesi dan privileging perawat anestesi di RSUD Wirosaban Kota Yogyakarta. Metode : Metode dan rancangan dalam penelitian ini deskriptif kualitatif, pengumpulan menggunakan observasi lapangan dan wawancara mendalam sertra Diskusi Kelompok Terfokus. Hasil penelitian : Implementasi regulasi belum optimal contoh belum ada Surat Ijin Praktek dokter spesialis anestesi. b) Manajemen RS belum menetapkan payung hukum pelayanan anestesi secara aman. c) Peraturan internal rumah sakit belum optimal seperti belum ada Standar Operasional Prosedur (SOP) pelayanan anestesi oleh dokter residen atau perawat. d) Kewenangan perawat pada saat ini adalah membantu dokter residen anestesi dalam pelayanan anestesi pada pasien. e) Tekanan kekuatan organisasi profesi sangat dominan, sehingga semua kegiatan dokter spesialis anestesi diatur sepenuhnya oleh IDSAI. f) Reward system belum sesuai dengan harapan profesi spesialis anestesi seperti sangsi, penghargaan dan ketegasan kompensasi. Kesimpulan: Berdasarkan hasil penelitian bahwa penerapan regulasi belum efektif. Privileging perawat anestesi belum terarah. Faktornya adalah peran pemerintah, peran RS, peran organisasi IDSAI dan IPPAI belum efektif dalam menjalankan fungsinya

Background : In the contexts of good governance, government has an important role in regulatory for guaranteeing patient safety. Medical Practice Law No. 29 year 2004 and Health Ministry Rule No. 512/MENKES/PER/IV/2007 declare that to get lisince practice, specialist has to get lisence and recommendation Indonetion Anaesthesia Specialist Doctor Association (IDSAI). Hospital licensing for public and private hospital is one of the regulatory activity, primary delegated to district level the shift of responsibility, i.e from mainly the central level to district level. Privileging shows to implementation of lisence, health facilities and health personnel. Regulatino is very important in implementation so that resposibility can be explainned clearly. Objective : This study a aimed to find out implementation level anesthesia service regulation and anaesthesia nurse privileging in Wirosaban district hospital Yogyakarta. Method : The method and design of study was discriptive qualitative study. Data collected using field observation, in depth intervew, and Focus Group Discution. Result : Implementation of regulation was not optimum yet, e.g.a 1) There was no lisence for anaesthesia specialist doctor practice. 2) No law darape over anaesthesia service. 3) No optimum internal rule in hospital, such as opertional prosedure standard of anaesthesia service by resident or nurse. 4) Nurse competence was just to assist resident in providing anaesthesia service. 5) Dominat pressure of professional organozation to arrange all activities of specialist. 6) Inapprropriete reward system. Concloution: Based on result of study, It is concludes that implementation of regulation is not effective in Wirosaban hospital. Privileging of anaesthesia nurse is not guided yet that caused by the rule of goverment, hospital, Indonesian Anaesthesia Spesialist Doctor Association (IDSAI), Indonesian Anaesthesia Nurse Asociation (IPAI) that are not effective in implement their functions.
