Hubungan antara kadar serum Albumin dan jumlah CD4+ pada wanita lanjut usia sehat di Paguyuban Lanjut Usia Aisyiyah Kota Gede Yogyakarta

Pertambahan usia menyebabkan terjadinya perubahan sensasi dan persepsi terhadap makanan serta perubahan struktur gigi dan jaringan ikat mulut. Perubahan ini sangat mempengaruhi kehidupan lanjut usia dan sering menimbulkan gangguan psikososial seperti depresi, apatis dan isolasi sosial. Hal ini diperberat oleh pengahasilan yang minim dan timbulnya perasaan menyendiri. Interaksi faktor-faktor tersebut semakin memperbesar penurunan asupan makanan, sehingga rentan terjadi malnutrisi. Malnutrisi energi protein merupakan penyebab utama imunodefisiensi, hal ini disebabkan sel imun mempunyai ketergantungan yang tinggi terhadap energi dan asam amino yang diperlukan saat pemisahan sel dan sintesa protein. Tujuan penelitian. Untuk mengetahui hubungan antara kadar serum albumin dan jumlah CD4+ pada lanjut usia sehat. Metode penelitian. Penelitian dilakukan secara potong lintang pada usia lanjut sehat yang tinggal di rumah dan tergabung dalam satu paguyuban lansia, pada bulan Desember 2006. Hasil. Jumlah keseluruhan subyek penelitian adalah 60 orang yang memenuhi kriteria penelitian dan setuju mengikuti penelitian, dengan rerata umur 64,4 ± 2,97 tahun. Umur terendah 60 tahun dan umur tertinggi 71 tahun. Kadar albumin rerata 4,11 ± 0,45 g/dl , dengan kadar terendah 3,12 g/dl dan kadar tertinggi 4,97 g/dL. Didapatkan 6 subyek (10%) dengan kadar albumin kurang dari normal ( < 3,5 g/dl ). Jumlah CD4+ rerata 767,22 ± 232,95/mm3 dengan kadar terendah 312/ mm3 dan kadar tertinggi 1261/ mm3 . Hasil analisa multipel regresi linear didapatkan kadar albumin (p<0,001; 95% IK 255,56 – 435,18), LLA (p=0,016; 95% IK:1,89 – 18,06) dan umur (p=0,038 ; 95% IK -25,95 - -0,78) yang berhubungan dengan jumlah CD4+. Kesimpulan. Terdapat hubungan positif antara kadar serum albumin dan jumlah CD4+ pada wanita lanjut usia sehat di Paguyuban Lanjut Usia Aisyiyah Kota Gede Yogyakarta.
Background. Aging is a natural process that happen to all of creatures. Growing age causes changes of sense perception to food and changes of teeth structure and oral tissue. These changes affect the life ederly persons and often cause phsycosocial problem such as depression , apathetic and social isolation. These changes aggravated of the low income and lonelyness . The interraction of these factor causes decreasing food intake, so that they are succeptible to malnutrition. The energy and protein malnutrition is the main cause of immunodeficiency, because immune cell has high independency to energy and amino acid to mitosis and protein synthesis. Objective. To know the association between plasma level of albumin and the number of CD4+ of the healthy ederly person. Methods. The research was conducted in cross-sectional design of elderly person who live in their own homes and join in elderly club in December 2006th. Result. The whole subjects of the research were 60 person who fulfilled the research criteria and aggree to participated the research. Their average age was 64.4 ± 2. 97 years old. The lowest was 60 years old and the highest was 71 years old. The average of albumin plasma level was 4.11± 0.45 g/dl. The lowest was 3.12 g/dl and the highest was 4.97 g/dl. It is found that the plasma levels of albumin of 6 person (10% of the subjects) were under normal (< 3.5 g/dl ). Their average of CD4+ was 767.22 ± 232.95/ mm3. The lowest was 312/ mm3 and the highest was 1261/ mm3 . In multiple regressioned lenearly analysis there were plasma level of albumin(p<0,001 CI 95% 255.56 – 435.18), MAC (p=0.016; CI 95% 1.89 – 18.06) and age (p=0.032 CI 95% -27.69 - -1.27) have association with the number of CD4+. Conclusion. There was a positive association between plasma level of albumin and the number of CD4+ in the woman healthy elderly person at the Aisyiyah women’s club Kota Gede Yogyakarta.
