Hubungan kadar matriks Metaloproteinase 3 (MMP-3) serum dengan kadar High Sensitive C-Reactive Protein Serum pada Osteoartritis Lutut

Latar belakang Osteoartritis (OA) adalah salah satu bentuk dari penyakit muskulo skeletal yang sering terjadi dan merupakan penyebab disabilitas terbesar kedua di dunia. Matriks metaloproteinase (MMP) penting untuk perubahan matriks tulang dan memainkan peranan yang penting di dalam kondisi patologis seperti OA. Matriks metaloproteinase merupakan enzim proteolitik yang dapat mendegradasi komponen matriks ekstraseluler. High Sensitive C-Reactive Protein (HS-CRP) adalah respon fase akut protein dalam darah yang mengindikasikan tingkat inflamasi dan dipertimbangkan sebagai salah satu indikator terbaik pada respon fase akut terhadap penyakit infeksi atau penyebab lain kerusakan jaringan dan inflamasi. Tujuan Untuk mengetahui hubungan antara kadar MMP 3 serum dengan kadar HS-CRP serum pada penderita OA lutut. Metode Penelitian ini menggunakan metode studi potong lintang dengan subyek penelitian ini adalah penderita OA yang baru terdiagnosis dan belum memperoleh terapi sebelumnya yang kontrol di Poliklinik Penyakit Dalam RS Dr. Sardjito, Jogjakarta. Penderita yang memenuhi kriteria dilakukan pemeriksaan kadar MMP-3 serum dan kadar HS-CRP serum setelah itu dilakukan analisis data. Dilakukan uji korelasi Spearman untuk menganalisis hubungan kadar MMP-3 serum dan kadar HS-CRP serum. Batas kemaknaan α=0,05 (95% CI). Kekuatan/power (1-β) 90% (β=10%). Hasil Didapatkan 40 orang subyek, terdiri dari 11 pria dan 29 wanita. Rerata kadar HS-CRP serum sebesar 0,569 ± 1,056 mg/dl dan rerata kadar MMP-3 serum adalah 18,18 ± 21 ng/ml. Hubungan kadar MMP-3 serum dan kadar HS-CRP serum lemah dan tidak bermakna (r= - 0,187; p=0,248). Simpulan Dari hasil penelitian menunjukan bahwa kadar MMP-3 serum tidak berhubungan dengan kadar HS-CRP serum.

Background, Osteoarthritis (OA) is one of the most musculoskeletal disease and the 2nd place to causes of disability in the whole of world. Matrix metalloproteinase (MMP) is an important thing due to matrix turn over and has main role in pathological conditions like OA. Matrix metalloproteinase is a proteolytics enzyme to degrade the extracellular matrix components. High Sensitive C-Reactive Protein (HS-CRP) is acute phase proteins response in blood which indicates inflammation levels and considered as the best indicator for acute phase response due to infection diseases or other tissue damage and inflammation. Objective, The aim of this study is to determine the association between MMP-3 serum levels and HS-CRP serum levels on patients with knee OA. Method, The method of this study is cross sectional study. The subjects are new patients from outpatients internal medicine department, Dr. Sardjito General Hospital, Jogjakarta which diagnosed by ACR 1986 criteria and never received medicine before. After they were eligible for the study the patient’s get blood were examined for MMP-3 and HS-CRP serum levels. Spearman statistic analysis ranked correlations performed to analyze the association between MMP-3 and HS-CRP serum levels. The significant level was α=0.05 (95% CI). The power (1- β) for this analysis was 90% (β=10%). Results 40 patients eligible for this study were 11 men and 29 women. From the statistical analysis mean of HS-CRP serum levels was 0.569 ± 1.056 mg/dl and MMP-3 serum levels 18.18±21 ng/ml. From Spearman rank correlation showed weak correlations and no significant association between MMP-3 and HS-CRP serum levels (r= - 0.187; p=0.248). Conclusion The study showed there was no association between MMP-3 and HSCRP serum levels.
