Tujuan laporan kasus ini ingin mengetahui hasil perawatan gigi insisivus sentralis kanan maksila fraktur 1/3 mahkota dan fraktur apikal pasca apikoektomi menggunakan Mineral TriOxide Agregate (MTA) dan restorasi mahkota jaket porselin fusi metal dengan inti pasak tuang dalam mengembalikan fungsi gigi seperti semula. Penderita laki-laki berumur 30 tahun datang ke klinik Konservasi Gigi FKG UGM ingin merawat gigi depan kanan atas yang patah akibat jatuh dari sepeda motor kurang lebih 1 tahun yang lalu. Gigi terasa sakit, terutama bila digunakan untuk minum dingin gigi terasa linu,agak goyah dan terasa maju, tapi belum pernah ada rasa sakit spontan. Berdasarkan pemeriksaan subjektif, objektif, dan radiografis diperoleh diagnosis gigi insisivus sentralis kanan maksila adalah gigi nekrosis pulpa, fraktur 1/3 mahkota dan apikal dengan lesi periapikal disertai periodontitis dan labioversi. Perawatan diawali perawatan saluran akar satu kunjungan, berikutnya dilanjutkan apikoektomi disertai pengisian retrograd menggunakan MTA kemudian restorasi mahkota jaket porselen fusi metal dengan inti pasak tuang. Hasil evaluasi secara klinis pada waktu kontrol, gigi insisivus sentralis kanan maksila tidak menunjukkan adanya rasa sakit pada waktu palpasi dan perkusi, warna gingiva normal, dan warna mahkota jaket gigi insisivus sentralis kanan maksila serasi dengan warna gigi yang lain.

The purpose of this case report was to find out the treatment outcome of maxillary right central incisor suffering from 1/3 crown and apical fracture using Mineral Trioxide Aggregate (MTA) following apicoectomy and the restoration of porcelain fused to metal jacket crown with custom dowel core in reestablishing the tooth function. A-30-year-old male who had been fallen down from motorcycle accident presented for treatment to Conservative Dentistry Clinic Faculty of Dentistry Gadjah Mada University. The tooth was sensitive, particularly when consuming cold beverage, slight mobility and protrusive, however no spontaneous pain occurred. Based on subjective, objective and radiograph examination, the diagnosis of maxillary right central incisor was pulpal necrosis, 1/3 crown and apical fracture with periapical lesion along with periodontitis and labioversion. The sequence of the treatment procedures were carried out as followed: one visit root canal treatment, apicoectomy in conjunction with retrograde filling using MTA, and restoration of porcelain fused to metal jacket crown with custom dowel core. The clinical evaluation outcome of maxillary right central incisor showed that no tenderness to percussion and palpation, normal gingival color, and jacket crown color accordance with adjacent teeth.
