Evaluasi pelaksanaan program Puskesmas Santun Usila di Kota SIngkawang Kalimantan Barat

Latar belakang. Pemerintah telah membuat strategi pendekatan Puskesmas Santun Usila untuk meningkatkan pelayanan kesehatan kepada usia lanjut dan program ini telah diterapkan secara nasional. Di Kota Singkawang, program tersebut telah dijalankan sejak tahun 2004 dan belum pernah dievaluasi. Tujuan Penelitian. Penelitian ini bertujuan untuk mengetahui gambaran pelaksanaan program Puskesmas Santun Usila di Kota Singkawang. Metode Penelitian. Penelitian ini merupakan penelitian deskriptif, dengan menggunakan metode kualitatif. Subjek penelitian ini adalah petugas pelaksana pelayanan kesehatan usila di Puskesmas, pasien yang dilayani, Kepala Puskesmas, dan Kasi Kesehatan Keluarga Dinas Kesehatan Kota Singkawang. Instrumen penelitian yang digunakan adalah kuesioner, panduan wawancara mendalam, panduan diskusi kelompok terarah dan daftar tilik dokumen. Hasil. Program Puskesmas Santun Usila belum didukung oleh kebijakan dari Dinas Kesehatan Kota Singkawang maupun Puskesmas karena program tersebut belum menjadi prioritas. Koordinator program di Puskesmas mempunyai tugas rangkap dan sebagian besar belum mendapatkan pelatihan. Sarana khusus untuk pelayanan usila belum ada dan dana operasional program Puskesmas Santun Usila belum tersedia. Hal tersebut berdampak pada proses pelaksanaan program tersebut. Perencanaan belum optimal, sementara pengorganisasian program hanya dilakukan dengan membuat struktur organisasi dan menunjuk penanggungjawab program tanpa memberikan petunjuk teknis yang jelas. Pelaksanaan program dilakukan melalui pelayanan usila di poli umum dan pengawasan terhadap pelayanan usila belum dilakukan. Pelaporan pelayanan usila masih menyatu dalam registrasi pelayanan poli umum. Angka cakupan kunjungan usila ke Puskesmas di Kota Singkawang hanya 38,74%, masih di bawah target yang diharapkan yakni 70%. Meskipun demikian, semua usila yang berkunjung mendapatkan pelayanan kesehatan. Usila merasa puas terhadap pelayanan kesehatan di Puskesmas yang berkaitan dengan keramahan petugas, namun belum puas dengan waktu tunggu. Kesimpulan. Program Puskesmas Santun Usila telah dijalankan di Puskesmas Kota Singkawang, namun belum optimal karena belum menjadi program prioritas, hal ini berakibat pada cakupan kunjungan yang belum mencapai target.

Background. The government has formulated a strategic approach called elderly people friendly health center to enhance health services towards elderly people and the program has implemented nationwide. In Singkawang, the program has been conducted since 2004 and never been evaluated. Objective. The objective of this research was to get an overview of the implementation of elderly people friendly health center in Singkawang City. Method. This research was a descriptive research, using qualitative method. The research subjects were health officers in charge of health services for elderly people in health centers, elderly patients, head of health centers, and head of family health section of Singkawang City health office. The instruments employed in the research were questionnaires, in -depth interview guide, focus group discussion guide, and document checklist. Result. The program of elderly people friendly health center has not been supported by policies of Singkawang City Health Office and health centers because it has not been considered as a priority. The program coordinators in health centers had multi tasks and most of them were not well-trained. Operational fund for the program and special health service facilities for elderly were also unavailable. This obviously affected the process of the program implementation. The program planning was not optimal. The program organization was only conducted by making organizational structure and assigning the program coordinator without clear technical guides. The program implementation was conducted by treating the elderly patients in general policlinic, while monitoring of health sevices towards the elderly patients was not done. The reports on health services towards the elderly patients were still integrated in the registration on general policlinic. The coverage number of elderly patients’ visits to health centers in Singkawang City was only 38.74%, still under the expected target of 70%. Yet, all elderly patients got their treatments. The elderly patients were satisfied with the services of health centers, which were related to health officers’ hospitality. Nevertheless, they were unsatisfied with the long waiting time. Conclusion. Singkawang City has conducted the program of elderly people friendly health center but the implementation was not optimal because the program has not been a priority. It affected the coverage of elderly patients’ visits, which was still under the expected target.
